Second primary malignancies induced by radioactive iodine treatment of differentiated thyroid carcinoma - a critical review and evaluation of the existing evidence

Summary: There is growing concern about long-term side effects of differentiated thyroid cancer treatment, particularly radioactive iodine (RAI) therapy. However, studies on the subject have had inconsistent results. This review aims to provide updated evaluation of published evidence and elucidate the risk of second primary malignancies (SPMs), specifically secondary hematologic malignancies (SHMs), associated with RAI therapy. The results indicate that the relative effect of RAI vs. no RAI ranged from 1.14 to 1.84 for SPM, but most results were not statistically significant. For SHM, the reported relative effects ranged from 1.30 to 2.50, with 2/3 of the studies showing statistically significant results. Increased risk for SPM was observed with increasing cumulative RAI activity in most studies. However, the overall quality of evidence was low.

Low-Iodine Diet of 4 Days Is Sufficient Preparation for 131I Therapy in Differentiated Thyroid Cancer Patients

Summary: This study investigated whether a 4-day low-iodine diet (LID) is sufficient for adequate iodine depletion in differentiated thyroid cancer (DTC) patients preparing for I-131 therapy. The results showed no significant difference in the 24-hour urinary iodine excretion (UIE) between day 4 and day 7 of the LID. On day 4, 72.1% of DTC patients achieved adequate preparation, increasing to 82.0% on day 7. Compared to their regular diet, DTC patients had significantly lower nutrient intake during the LID.

Summary: This study evaluated long-term male fertility and semen quality after radioactive iodine treatment for differentiated thyroid carcinoma. Results showed that participants had normal long-term semen quality and the proportion with low semen quality did not differ significantly from the general population. The highest attributable risk proportion of RAI treatment was observed in hematological malignancies, but due to their low incidence among all DTC survivors, the absolute number of hematological malignancies was low.

Diagnostic performance of adult-based ATA and ACR-TIRADS ultrasound risk stratification systems in pediatric thyroid nodules: a systematic review and meta-analysis

Summary: The diagnostic performance of ACR-TIRADS and ATA in the pediatric population was found to be somewhat modest, with both systems showing moderate sensitivity and specificity. ACR-TIRADS had significantly higher specificity in category 5 nodules compared to ATA, but data for ATA were insufficient for comparison. Further large studies are needed for validation and potential improvements in the future.

Summary: This longitudinal study of pediatric DTC survivors demonstrated normal and stable median lumbar spine and femur BMD values after a median time of 17 and 23.5 years after diagnosis. However, compared with controls, a lower BMD was still found in 13.0% after prolonged follow-up despite intensive follow-up. These data do not provide convincing evidence in support of standard monitoring of bone mass among DTC survivors, but may be restricted to individual cases at low frequency.
