Vasoactive-Inotropic Score as a Determinant of Timely Initiation of Venoarterial Extracorporeal Membrane Oxygenation in Patients With Cardiogenic Shock

Junho Hyun et al.

Summary: The vasoactive-inotropic score (VIS) can serve as a potential marker for determining the initiation of hemodynamic support with venoarterial extracorporeal membrane oxygenation (VA-ECMO) in patients with cardiogenic shock, leading to better in-hospital outcomes. Factors predicting in-hospital death include extracorporeal cardiopulmonary resuscitation, age, pre-ECMO lactate, and high VIS values.

Early risk stratification in patients with cardiogenic shock irrespective of the underlying cause - the Cardiogenic Shock Score

Benedikt N. Beer et al.

Summary: This study aimed to develop a universal risk score, the Cardiogenic Shock Score (CSS), for all cardiogenic shock (CS) patients. The CSS showed better predictive ability for mortality compared to existing scores, regardless of the underlying cause. It has the potential to guide treatment decisions in CS.

Jeffrey M. Tyler et al.

Summary: Among 15 RCTs enrolling 2,500 AMICS patients over 21 years, key prognostic factors were variably reported with rates ranging from 0% to 93%. Standardizing data collection and reporting in clinical trials for cardiogenic shock may improve assessment of novel therapies for AMICS.

Laboratory Markers of Acidosis and Mortality in Cardiogenic Shock: Developing a Definition of Hemometabolic Shock

Jacob C. Jentzer et al.

Summary: This study retrospectively analyzed cardiogenic shock patients admitted to a cardiac intensive care unit from 2007 to 2015. The results showed that the severity of acidosis on admission was positively associated with in-hospital mortality, and severe acidosis was related to shock severity and organ failure. Admission lactate levels had the same predictive ability for in-hospital mortality as a comprehensive measure of acidosis.

Mechanical Left Ventricular Unloading in Patients Undergoing Venoarterial Extracorporeal Membrane Oxygenation

E. Wilson Grandin et al.

Summary: Among adults receiving VA-ECMO, mechanical unloading (MU) with intra-aortic balloon pump (IABP) or percutaneous ventricular assist device (pVAD) was associated with lower in-hospital mortality and increased complications. Compared to pVAD, MU with IABP had similar mortality and lower complication rates.

Society for cardiovascular angiography and intervention shock classification predicts mortality after out-of-hospital cardiac arrest

Dhruv Sarma et al.

Summary: This study investigated the association between shock severity, as defined by the SCAI Shock Classification, and in-hospital mortality and neurological outcome in comatose OHCA patients undergoing TTM. The study found that higher shock severity was associated with increased in-hospital mortality and a lower likelihood of good neurological outcome.

Predictors of Mortality in Patients Treated with Veno-Arterial ECMO for Cardiogenic Shock Complicating Acute Myocardial Infarction: a Systematic Review and Meta-Analysis

Shahmir Sohail et al.

Summary: This meta-analysis identified several factors associated with higher mortality in patients undergoing VA-ECMO for CS complicating AMI, including failure to achieve TIMI III flow, left main culprit, older age, high BMI, renal dysfunction, increasing lactate, prothrombin activity < 50%, VA-ECMO implantation after revascularization, and non-shockable ventricular arrythmias. These results provide clinicians with guidance in selecting appropriate patients for VA-ECMO treatment.

Jacob C. Jentzer et al.

Summary: The prevalence of shock in the CICU has increased over time, with a significant prevalence of mixed cardiogenic-septic shock. The etiology of cardiogenic shock has changed over the last decade, with heart failure overtaking acute coronary syndrome as the most common cause of cardiogenic shock in the CICU.

Mortality Predictors in Elderly Patients With Cardiogenic Shock on Venoarterial Extracorporeal Life Support. Analysis From the Extracorporeal Life Support Organization Registry*

Mariusz Kowalewski et al.

Summary: The study analyzed predictors of unfavorable outcomes in elderly patients (>= 70 yr) undergoing venoarterial extracorporeal membrane oxygenation for refractory cardiogenic shock. Despite high in-hospital mortality, the use of ECMO in elderly patients is still considered, as increasing age itself was not associated with higher mortality.

Trends in Extracorporeal Membrane Oxygenation Application and Outcomes in Korea

Eun Jin Jang et al.

Summary: The utilization of ECMO has increased significantly over time, resulting in a higher number of procedures and related costs. Despite this, in-hospital mortality rates have improved, particularly for patients undergoing lung transplantation and experiencing respiratory failure. Hospitals with higher case volumes demonstrated better patient outcomes.

Differential Prognostic Implications of Vasoactive Inotropic Score for Patients With Acute Myocardial Infarction Complicated by Cardiogenic Shock According to Use of Mechanical Circulatory Support*

Ki Hong Choi et al.

Summary: This study found that in patients with acute myocardial infarction complicated by cardiogenic shock, a high Vasoactive Inotropic Score was associated with higher in-hospital and follow-up mortality. The predictive value of Vasoactive Inotropic Score for in-hospital mortality was significantly better in acute myocardial infarction patients with cardiogenic shock receiving medical treatment alone compared to those treated with mechanical circulatory support like intra-aortic balloon pump or extracorporeal membrane oxygenation.
