De novo and relapsing necrotizing vasculitis after COVID-19 vaccination

Summary: Five cases of severe necrotizing vasculitis following SARS-CoV-2 RNA-based vaccine were described, including four cases of relapsing ANCA vasculitis and one case of de novo PAN in a patient with quiescent chronic hepatitis B. Ten additional cases were reported in the French national pharmacovigilance database, with six patients having ANCA-associated vasculitis and four patients having PAN (with first symptoms occurring on average 19 days after vaccination). Five of these 10 patients developed kidney dysfunction. In conclusion, COVID-19 vaccines may be associated with de novo or recurrent ANCA vasculitis or PAN, and attention should be given to patients with known ANCA vasculitis or a history of hepatitis B infection.

Safety and Efficacy of the Common Vaccines against COVID-19

This article aims to conduct a systematic review to evaluate the safety and effectiveness of COVID-19 vaccines by analyzing clinical data and mass vaccination data. It is found that inactivated vaccines have lower adverse reactions, with an efficacy of approximately 60%, while adenovirus-vectored vaccines have an efficacy of 65%, and mRNA vaccines have an efficacy of 90%. RNA-based vaccines are considered promising and important during a pandemic, but further improvements are required, such as the need for booster doses to maintain immunity and the potential benefits of heterologous prime-boost immunization.

This case study describes a 45-year-old female patient who developed acute kidney injury after receiving the BNT162b2 SARS-CoV 2 vaccine, along with macular rash. Treatment with corticosteroids resulted in clinical improvement and recovery of kidney function, indicating that acute reactions to vaccines can occur. Healthcare providers and patients need to be aware of these potential side effects.
