Summary: COVID-19, caused by SARS-CoV-2, is a global pandemic with complications including acute lung injury and acute respiratory distress syndrome due to exaggerated immune response. A series of case reports showed marked improvements in symptoms, clinical, and radiological outcomes in COVID-19 pneumonia patients treated with a combination of doxycycline and colchicine, suggesting the need for large-scale prospective studies.

COVID-19 and Risk of Acute Ischemic Stroke and Acute Lung Injury in Patients With Type II Diabetes Mellitus: The Anti-inflammatory Role of Metformin

Summary: Metformin therapy in type 2 diabetes patients with COVID-19 was associated with a lower risk of acute ischemic stroke (AIS). Metformin-treated patients showed lower levels of ferritin, CRP, LDH, and D-dimer compared to non-metformin treated patients, as well as lower lung CT scan scores. Further studies are needed to explore the relationship between AIS in COVID-19 diabetic patients and metformin therapy.

Case Report: Hyperbilirubinemia in Gilbert Syndrome Attenuates Covid-19-Induced Metabolic Disturbances

Summary: This study described a case of a 47-year-old man with Gilbert syndrome (GS) who was diagnosed with Covid-19 pneumonia. The patient with GS showed a rapid clinical recovery and shorter hospital stay compared to other Covid-19 patients, suggesting that hyperbilirubinemia may have a protective effect. The data suggests that the higher resistance against Covid-19 observed in the GS patient may be due to the antioxidant, anti-inflammatory, and antiviral effects of unconjugated bilirubin.

Some antidiabetic agents may reduce SARS-CoV-2 severity through modulation of the ACE2 receptor expression.

Estrogens have significant anti-inflammatory and immuno-modulatory effects in Covid-19, reducing SARS-CoV-2 infectivity. Future clinical trials and prospective studies will help elucidate and confirm the protective effect of estrogen against Covid-19.

Anti-heart failure medications like digoxin and carvedilol may attenuate severity of HF by reducing infectivity of SARS-CoV-2 and preventing the development of cytokine storms in severely affected Covid-19 patients.
