Impact of an Antimicrobial Stewardship Program-bundled initiative utilizing Accelerate Pheno™ system in the management of patients with aerobic Gram-negative bacilli bacteremia

Summary: A study was conducted to compare the management of non-critically ill patients with GNB BSI before and after the implementation of a bundled initiative. The initiative included clinical decision algorithm dissemination, ACC testing on all GNB isolated from blood cultures, real-time communication of results to ASP, and prospective audit with feedback by ASP. The implementation led to improvements in time from Gram stain to AST, time to definitive therapy, total duration of antibiotic therapy, and hospital length of stay without increasing readmission rates.

Maddalena Giannella et al.

Summary: The impact of five interventions on managing Enterobacteriaceae bloodstream infection (E-BSI) was reviewed. Current data only support short treatment duration in non-immunocompromised patients with E-BSI. Further studies are needed to investigate the use of imaging, follow-up blood cultures, extended/continuous infusion β-lactams, and infectious diseases consultation in E-BSI patients.

Impact of follow up blood cultures on outcomes of patients with community-onset gram-negative bloodstream infection

Rajiv Amipara et al.

Summary: The role of follow up blood cultures in the management of gram-negative bloodstream infection remains controversial. However, obtaining FUBC was independently associated with reduced mortality in patients with GN-BSI, particularly in those who were older, had chronic comorbidities, acute severity of illness, and received appropriate empirical antimicrobial therapy.
