Measuring the impact of an empiric antibiotic algorithm for pulmonary exacerbation in children and young adults with cystic fibrosis

Summary: Antimicrobial stewardship is effective in reducing unnecessary use of broad-spectrum antibiotics and improving the quality of care for CF patients. Initiating an empiric antibiotic algorithm led to a significant increase in antibiotic use consistent with the algorithm, improved documentation of justification for broad-spectrum antibiotics, and increased use of ID consults. This highlights the benefits of standardizing care and promoting collaboration among healthcare professionals.

Inflammation in children with cystic fibrosis: contribution of bacterial production of long-chain fatty acids

Summary: Long-chain fatty acid biosynthesis pathways were upregulated in follow-up samples compared to the end of treatment, and Achromobacter xylosoxidans and Escherichia sp. were more prevalent in follow-up. LCFAs may be associated with persistent infection of opportunistic pathogens. Future studies should investigate the role of bacterial production of LCFAs in CF patients' transition from baseline wellness to PEx.

Importance of beta-lactam pharmacokinetics and pharmacodynamics on the recovery of microbial diversity in the airway of persons with cystic fibrosis

Summary: The study found that the pharmacokinetic target attainment of beta-lactam antibiotics in children with cystic fibrosis was associated with suppressed recovery of airway microbial diversity following treatment. Patients with sufficient antibiotic levels had decreased alpha diversity at follow-up, while those with insufficient levels showed an increase in diversity during the same period. Inadequate beta-lactam levels were also linked to a higher frequency of fluoroquinolone resistance.
