Results of a Multi-Institutional Phase 2 Clinical Trial for 4DCT-Ventilation Functional Avoidance Thoracic Radiation Therapy

Summary: Functional avoidance radiation therapy reduces the occurrence of radiation pneumonitis by designing radiation therapy plans that minimize doses to functional areas of the lung. A multicenter study using 4DCT-ventilation functional imaging demonstrated that this treatment approach met the criteria for phase 2 trials, suggesting further evaluation in a phase 3 study is warranted.

Quantitative assessment of ventilation-perfusion relationships with gallium-68 positron emission tomography/computed tomography imaging in lung cancer patients

Summary: This study used a new functional imaging approach, Ga-68 PET/CT, to quantify the ventilation/perfusion relationships in lung cancer patients and compared it with the diffusing capacity of the lung for carbon monoxide (D-LCO). The results showed a moderate correlation between Ga-68 PET/CT-measured percent gas exchanging lung volume and D-LCO, suggesting that the ventilation/perfusion imaging provided complementary information and served as a reasonable surrogate for gas exchange in lung cancer patients.

Functional avoidance-based intensity modulated proton therapy with 4DCT derived ventilation imaging for lung cancer

Summary: The study evaluated the dosimetric gains of functional avoidance-based proton treatment planning compared to conventional treatment planning, showing significant dose reduction to lung structures without increasing dose to Organ at Risk (OAR) structures. Functional proton plans did not exhibit significant dose degradation to OARs or clinical target volumes when considering interplay effect. Additionally, NTCP calculation indicated a further reduction in the risk of pulmonary complications with functional based IMPT.
