Changes in sexual function and vaginal topography using transperineal ultrasound after vaginal laser treatment for women with stress urinary incontinence

Summary: The study aimed to evaluate changes in sexual function and vaginal topography in stress-incontinent women treated with Er:YAG vaginal laser using 3-D transperineal ultrasound. Significant improvements were observed in vaginal shrinkage and female sexual function after treatment. Symptomatic improvement was noted in the majority of women who underwent the laser treatment.

Summary: This narrative review evaluated the efficacy and safety of new minimally invasive solutions for stress urinary incontinence (SUI) treatment, specifically the microablative fractional carbon dioxide laser and the non-ablative Erbium-YAG laser. The short-term follow-up results showed significant subjective improvement, although cure rates were low and longer-term effects require further investigation.

Second-Line Surgical Management After Midurethral Sling Failure

Summary: The midurethral sling is widely used for stress urinary incontinence, but has a failure rate of 5%-20%. There are no clear guidelines for second-line management, but options include bulking agent injections, tape shortening, repeat MUS, PVS, and colposuspension. Repeat MUS is the most commonly used second-line surgical method, while PVS and colposuspension without synthetic mesh are preferred for patients with prior mesh-related complications. A readjustable sling may be a better option for patients with detrusor underactivity.
