Waiting Time for Second Kidney Transplantation and Mortality

Alexander Kainz et al.

Summary: The study results indicate that second kidney transplantation is associated with longer survival time compared to remaining on the waiting list after 10 years of follow-up. However, this survival difference diminishes with longer waiting times after the loss of the first graft.

Association between kidney retransplantation and survival according to age in the French national cohort of dialysis patients

Sophie Girerd et al.

Summary: This study aimed to evaluate the impact of age at the time of returning to dialysis on the association between second kidney transplantation (2KT) and survival. The findings showed that 2KT was associated with better survival compared to remaining on dialysis after the first graft failure. However, this survival benefit decreased with increasing age.

Summary: The year 2020 presented challenges for kidney transplantation, with a slight decrease in the total number of transplants and a significant decline in living donor transplants due to the COVID-19 pandemic. Racial disparities in living donor transplant access persist. Pediatric kidney transplants were also impacted, with a decrease in the total number of transplants and a continued decrease in deceased donor transplants among pediatric candidates.

Summary: This study investigated the use of immunosuppressants in patients with kidney transplant failure and found that prolonged use of immunosuppressants for more than 2 years was not associated with an increased risk of death or hospitalized infection. However, continued use of immunosuppressants did not prevent rejection of the failed allograft or an increase in anti-HLA antibodies.

Summary: The proportion and risk of re-KT in older adults have increased over time, but outcomes have improved significantly. These findings support increasing access to re-KT in older adults, although approaches to guide selection and management in those with graft failure need to be explored.

Kidney Retransplant: Not Too Old for a Second Chance

Clara Pardinhas et al.

Summary: This study compared major clinical outcomes between young and elderly patients undergoing retransplant, as well as between first and second kidney transplant in recipients older than 60. The results showed that the major clinical outcomes of retransplant in the elderly were similar to those of their younger counterparts with a second graft and older patients with a first graft.

Summary: This study investigated the efficacy and safety of imlifidase in converting a positive crossmatch test to negative in highly sensitized patients, allowing them to undergo kidney transplantation. Results showed that majority of transplanted patients achieved negative crossmatch within 24 hours after imlifidase treatment, with good kidney function and graft survival at 6 months.

Association Between Donor-Recipient Biological Relationship and Allograft Outcomes After Living Donor Kidney Transplant

S. Ali Husain et al.

Summary: The study found that living donor kidney transplants from donors biologically related to their recipients had higher rates of allograft failure than transplants from donors unrelated to their recipients after HLA matching was accounted for. This association was mainly observed in recipients without cystic kidney disease and transplants from African American donors. Further research is needed to explore the factors contributing to this finding.

The failing kidney allograft: A review and recommendations for the care and management of a complex group of patients

Michelle Lubetzky et al.

Summary: The return to dialysis after allograft failure is complicated by the increasing number of patients seeking repeat transplantation and the need for lower levels of immunosuppression. There is limited data on how to transition off immunosuppression and a lack of randomized controlled trials to guide management strategies. A shared-care model is proposed to improve outcomes and reduce morbidity in this vulnerable patient group.

Significance of HLA-DQ in kidney transplantation: time to reevaluate human leukocyte antigen-matching priorities to improve transplant outcomes? An expert review and recommendations

Anat R. Tambur et al.

Summary: The importance of HLA matching in kidney allocation algorithms, especially in the US, has decreased due to the introduction of modern immunosuppression, aiming to reduce ethnic/racial disparities in access to transplantation. However, chronic antibody-mediated rejection, primarily caused by de novo antibodies against mismatched donor HLA, remains a leading cause of graft loss, affecting the availability of second transplants for sensitized patients. Recommendations include examining the impact of donor-recipient HLA class II and reconsidering best practices to reduce inequalities while optimizing patient outcomes.

Highly Sensitized Patients Are Well Served by Receiving a Compatible Organ Offer Based on Acceptable Mismatches

Sebastiaan Heidt et al.

Summary: The Eurotransplant Acceptable Mismatch (AM) program has successfully increased transplantation opportunities for highly sensitized kidney patients, allocating compatible donor kidneys based on recipient's HLA antigens and predefined acceptable antigens. Through this program, roughly 80% of patients have received organ offers, with studies showing similar rejection rates and long-term graft survival compared to non-sensitized patients. Collaboration and extension of the donor pool could further benefit highly sensitized patients who do not receive organ offers in a timely manner.
