Barriers to the Use of Active Surveillance for Thyroid Cancer Results of a Physician Survey

David T. Hughes et al.

Summary: Despite the belief among most physicians that active surveillance is an appropriate management option for thyroid cancer, only a minority of them actually use it in their practice. Barriers reported by physicians include patient resistance, concerns about loss to follow-up, patient anxiety, and worries about medical malpractice lawsuits. Addressing these barriers is crucial for improving the uptake of active surveillance.

Indications and Strategy for Active Surveillance of Adult Low-Risk Papillary Thyroid Microcarcinoma: Consensus Statements from the Japan Association of Endocrine Surgery Task Force on Management for Papillary Thyroid Microcarcinoma

Iwao Sugitani et al.

Summary: The management of patients with low-risk papillary thyroid microcarcinoma (PTMC; T1aN0M0) has become an important clinical issue, with the adoption of active surveillance (AS) to avoid overtreatment gradually increasing worldwide. However, not all PTMCs are suitable for AS, and the Japan Association of Endocrine Surgery has produced consensus statements regarding indications and strategies for AS in adult patients diagnosed with low-risk PTMC. These statements provide guidelines for AS and recommend immediate surgery for certain high-risk features in PTMC patients.

Adoption of Active Surveillance for Very Low-Risk Differentiated Thyroid Cancer in the United States: A National Survey

Susan C. Pitt et al.

Summary: The study aimed to understand factors associated with the adoption of active surveillance (AS) for thyroid cancer. Results showed that nonadopters were less likely to work in academic settings, see a high number of thyroid cancer patients, be aware of AS, use applicable guidelines, have resources or motivation for AS, while adopters were more likely to have awareness, resources, and motivation for AS.

Presence of TERT ± BRAF V600E mutation is not a risk factor for the clinical management of patients with papillary thyroid microcarcinoma

Jeonghun Lee et al.

Summary: The study showed that TERT +/- BRAF V600E mutations in patients with papillary thyroid microcarcinomas are not associated with aggressive clinical, radiological, or pathological features, and the presence of lymph node metastasis is not influenced by mutation status.
