Active Surveillance Versus Thyroid Surgery for Differentiated Thyroid Cancer: A Systematic Review

Roger Chou et al.

Summary: This systematic review examines the benefits and harms of active surveillance vs. immediate surgery for low-risk differentiated thyroid cancer (DTC). The results suggest that active surveillance and immediate surgery may be associated with similar mortality, risk of recurrence, and other outcomes in low-risk small DTC patients, but the methodological limitations prevent strong conclusions.

Association of Total Thyroidectomy or Thyroid Lobectomy With the Quality of Life in Patients With Differentiated Thyroid Cancer With Low to Intermediate Risk of Recurrence

Wanna Chen et al.

Summary: This study compared the HRQOL of patients with DTC undergoing different surgeries longitudinally, and found that there was no significant association between the extent of surgery and HRQOL in patients with low to intermediate risk of recurrence. TL surgery may be preferred if better HRQOL is requested in the short term.

The epidemiological landscape of thyroid cancer worldwide: GLOBOCAN estimates for incidence and mortality rates in 2020

Margherita Pizzato et al.

Summary: The incidence rates of thyroid cancer in 2020 were higher in high human development index countries compared to low and medium human development index countries. However, mortality rates were relatively similar across different settings. This suggests an overdiagnosis effect in many countries and confirms the relevance of thyroid cancer overdiagnosis as a global public health problem.

Papillary Thyroid Microcarcinoma: Active Surveillance Against Surgery. Considerations of an Italian Working Group From a Systematic Review

Giuseppina Orlando et al.

Summary: This review examines the most recent findings on active surveillance (AS) for papillary thyroid microcarcinoma published from 2017 to 2020. The study includes nine articles and finds that tumor volume increased by 2.7% to 23.2%, lymph node metastases occurred in 1.3% to 29% of cases, and quality of life was improved in the articles that addressed this topic. However, the evidence is considered low due to the retrospective and uncontrolled nature of most of the studies.

Low-Risk Papillary Thyroid Cancer: Treatment De-Escalation and Cost Implications

Christina M. Paluskievicz et al.

Summary: This study investigated the impact of the American Thyroid Association's updated guidelines on the practice patterns of low-risk papillary thyroid cancer treatment. The analysis of data revealed a decrease in the utilization of surgery and radioactive iodine treatment for low-risk cases in accordance with the guidelines, and these changes resulted in significant cost savings.

Indications and Strategy for Active Surveillance of Adult Low-Risk Papillary Thyroid Microcarcinoma: Consensus Statements from the Japan Association of Endocrine Surgery Task Force on Management for Papillary Thyroid Microcarcinoma

Iwao Sugitani et al.

Summary: The management of patients with low-risk papillary thyroid microcarcinoma (PTMC; T1aN0M0) has become an important clinical issue, with the adoption of active surveillance (AS) to avoid overtreatment gradually increasing worldwide. However, not all PTMCs are suitable for AS, and the Japan Association of Endocrine Surgery has produced consensus statements regarding indications and strategies for AS in adult patients diagnosed with low-risk PTMC. These statements provide guidelines for AS and recommend immediate surgery for certain high-risk features in PTMC patients.

Papillary Thyroid Carcinoma in Ukraine After Chernobyl and in Japan After Fukushima: Different Histopathological Scenarios

Tetiana I. Bogdanova et al.

Summary: Ukrainian radiogenic PTCs exhibited higher invasiveness and tumor growth patterns compared to sporadic PTCs, while no significant differences were observed in Japanese groups. These findings strongly suggest a non-radiogenic etiology of PTC in Fukushima and other Prefectures.

Summary: The study found that the incidence of clinically relevant thyroid cancers has not significantly changed over 80 years, while the rapid increase in thyroid cancer incidence is being driven by indolent thyroid cancers.

Summary: This study analyzed international trends of thyroid cancer incidence by major histological subtypes based on data collected by IARC from 1998 to 2012. The results showed that papillary thyroid cancer was the main contributor and the only subtype that systematically increased in all countries, with large variability between countries.

Risk Factors for Central and Lateral Lymph Node Metastases in Patients With Papillary Thyroid Micro-Carcinoma: Retrospective Analysis on 484 Cases

Yijie Huang et al.

Summary: PTMC patients have a significant incidence of lymph node metastases. Risk factors for CLNM include age, gender, and tumor size, while extrathyroidal extension and number of CLNM are strong indicators for LLNM. Identifying these factors can help tailor individual surgical interventions for PTMC patients.

The BRAFV600E Mutation Is Not a Risk Factor for More Aggressive Tumor Behavior in Radiogenic and Sporadic Papillary Thyroid Carcinoma at a Young Age

Liudmyla Zurnadzhy et al.

Summary: The frequency of BRAF(V600E) in young Ukrainian patients with radiogenic and sporadic papillary thyroid carcinomas increases with age, but does not confer a more aggressive course regardless of tumor etiology. Major clinicopathological differences between radiogenic and sporadic PTCs are observed among the BRAF(V600E)-negative tumors.

Papillary thyroid microcarcinoma: Is active surveillance always enough?

Rosie Sutherland et al.

Summary: The incidence of papillary thyroid carcinoma is increasing, with a focus on overdiagnosis and disease progression. While most cases follow a chronic disease course, some show more aggressive features. Current guidelines recommend against routine biopsy for nodules <= 10 mm, and active surveillance is recommended for low-risk PTMC.

Preoperative detection of the TERT promoter mutations in papillary thyroid carcinomas

Tomoe Nakao et al.

Summary: The study found a high concordance rate of TERT-p mutations between preoperative fine-needle aspiration samples and postoperative formalin-fixed and paraffin-embedded specimens in papillary thyroid carcinomas (PTCs) aged 55 years or older. The TERT-p mutation was significantly associated with age, tumor size, extrathyroidal extension, and the Ki-67 labelling index in both FNA and FFPE samples, indicating potential clinical utility in predicting disease aggressiveness and guiding PTC management.
