Immunological findings in patients with migraine and other primary headaches: a narrative review

Leonardo Biscetti et al.

Summary: Experimental findings suggest that neuroinflammatory mechanisms play a role in the pathophysiology of migraine, particularly involving the activation of the trigeminal pathway and inflammatory responses. Increased levels of pro-inflammatory cytokines and immune dysregulation in migraine patients support the involvement of neuroinflammation in migraine. Additional research is needed to clarify the role of neuroinflammation in cluster headache and tension-type headache.

Calcitonin Gene-Related Peptide (CGRP)-Targeted Monoclonal Antibodies and Antagonists in Migraine: Current Evidence and Rationale

Fred Cohen et al.

Summary: Calcitonin gene-related peptide (CGRP) is believed to be involved in the pathogenesis of migraine, and there are two classes of therapeutic agents targeting CGRP: CGRP-targeted monoclonal antibodies and small-molecule antagonists. Multiple clinical studies have shown the efficacy and tolerability of these treatments.

Identifying New Antimigraine Targets: Lessons from Molecular Biology

Lars Edvinsson et al.

Summary: Primary headaches, with migraines being the most common type, are believed to be influenced by a mismatch in the trigeminovascular system, resulting in sensitization and the release of CGRP. The research focuses on how to modify this system for future treatments by understanding the distribution and expression of neuropeptides and ion channels.

Headache and immunological/autoimmune disorders: a comprehensive review of available epidemiological evidence with insights on potential underlying mechanisms

Leonardo Biscetti et al.

Summary: There is evidence supporting the role of the immune system in headache pathogenesis, with changes in cytokine profile and lymphocyte subsets in headache patients. Immunological alterations may predispose some patients to immunological and autoimmune diseases, while pathogenic mechanisms of autoimmune disorders can favor the onset of headaches. Investigating the association between headache and immunological/autoimmune disorders can have implications in clinical practice.

Pathophysiological Bases of Comorbidity in Migraine

Claudia Altamura et al.

Summary: Despite being commonly accepted as a disorder of the nervous system with a genetic basis, migraine is comorbid with various medical conditions including neurological, psychiatric, cardio- and cerebrovascular, gastrointestinal, metaboloendocrine, and immunological conditions. The bidirectional comorbidity between migraine and these diseases involves complex mechanisms that may induce and promote abnormal excitability in the brain, inflammatory responses, and energy imbalances, resulting in the activation of defense systems. Treatment of migraine should involve a multidisciplinary approach to identify and address possible risk and comorbidity factors.

Migraine and neuroinflammation: the inflammasome perspective

Oguzhan Kursun et al.

Summary: Neuroinflammatory pathways, especially those involving inflammasome proteins, show promise as potential treatment targets and even biomarkers in migraine. The activation of inflammasomes can stimulate trigeminal neurons and contribute to the generation of migraine pain, making them an important focus in migraine research. The NLRP3 inflammasome, in particular, is well-studied and has been implicated in various inflammatory pain conditions, including migraine.

Summary: The study compared the expression levels of cytokines in the blood of patients with migraine and healthy individuals, revealing dysregulation of cytokine-coding genes in the blood of migraine patients. Certain cytokines showed higher expression in migraine cases, with INF-gamma being significantly higher, while others like CXCL8 were lower than in controls.
