Mycobacterium tuberculosis and SARS-CoV-2 Coinfections: A Review

Narjess Bostanghadiri et al.

Summary: Tuberculosis management during the COVID-19 pandemic has been neglected, leading to severe consequences for patients and health systems. This review found that coinfection of tuberculosis and COVID-19 results in severe symptoms and higher mortality rate, indicating the need for proper attention and management.

Mycobacterium tuberculosis and Pulmonary Rehabilitation: From Novel Pharmacotherapeutic Approaches to Management of Post-Tuberculosis Sequelae

Andreea-Daniela Meca et al.

Summary: Tuberculosis remains a global public health burden with high mortality rates. Little is known about the pulmonary sequelae and quality of life of patients who have completed anti-TB treatment. Understanding the immunologic and pathogenic mechanisms and developing new anti-TB drugs can guide the application of rehabilitation care. The use of rehabilitation programs for patients who have completed treatment can prevent an increase in mortality rates. Dissemination of pulmonary rehabilitation services and constant updating of protocols can strengthen the management of post-TB disease.

Clinical features and outcomes of COVID-19 admissions in a population with a high prevalence of HIV and tuberculosis: a multicentre cohort study

Arifa Parker et al.

Summary: This study describes the clinical features, laboratory findings, and outcomes of COVID-19 patients living with HIV and those co-infected with tuberculosis in three hospitals in Cape Town, South Africa. The study found that being overweight or obese was associated with an increased risk of mortality in COVID-19 hospital admissions.

Summary: This study investigated the role of HS-CRP in TB diagnosis and mortality prediction in HIV-positive patients. The results showed that malnutrition and elevated HS-CRP were associated with TB, and the combination of these factors was highly predictive of death.

Pulmonary tuberculosis screening in anti-retroviral treated adults living with HIV in Kenya

Jill K. Gersh et al.

Summary: In Kenyan PLHIV treated with IPT, tuberculosis prevalence was low at a median of 1.4 years after IPT completion. WHO symptoms screening, Xpert, and candidate biomarkers were insensitive for identifying pulmonary tuberculosis in antiretroviral-treated PLHIV.

Impact of hematocrit on point-of-care C-reactive protein-based tuberculosis screening among people living with HIV initiating antiretroviral therapy in Uganda

Sandra Z. Mwebe et al.

Summary: The study compared the diagnostic accuracy of unadjusted and hematocrit-adjusted POC CRP for culture-confirmed TB among PLHIV and found similar sensitivity and specificity between the two. Therefore, POC CRP may not require adjustment for hematocrit in TB screening. Larger studies may be needed to detect differences close to the clinically meaningful threshold.

Evaluation of Host Serum Protein Biomarkers of Tuberculosis in sub-Saharan Africa

Thomas C. Morris et al.

Summary: A study evaluated serum protein biomarkers discovered in primary care cohorts from Africa reapplied to patients from secondary care, discovering a nine-protein signature to distinguish tuberculosis from other diseases. This signature showed high sensitivity and specificity, indicating the potential for serum proteins as point-of-care screening tests for TB in African hospitals. Additionally, complement factor H was identified as a new biomarker for such signatures.

Summary: Tuberculosis is a global infectious disease with a complex pathogenesis. Recent studies have shown changes in oxidative parameters and antioxidant status in TB patients after anti-tubercular therapy. Monitoring TB based on oxidative stress and antioxidant status may be beneficial in disease management and progression.

Old biomarkers in tuberculosis management: are they still useful? a systematic review

Lara Fusani et al.

Summary: This systematic review focused on the potential usefulness of old biomarkers in the diagnosis and management of childhood tuberculosis. Biomarkers such as C-reactive protein, ferritin, erythrocyte sedimentation rates, and procalcitonin were found to have promising roles in the differential diagnosis and treatment monitoring of TB. Further studies, especially in children, are needed to fully implement the use of these old biomarkers in clinical practice.

Summary: This study identified a set of hematological parameters that significantly decreased during treatment in patients who turned culture negative, indicating their potential as predictors for treatment outcomes. Despite limitations in the study, these findings are valuable for further research on hematological profiles in predicting treatment efficacy.

Evaluation of the diagnostic performance of laboratory-based c-reactive protein as a triage test for active pulmonary tuberculosis

Thomas H. A. Samuels et al.

Summary: The study found that the CRP triage test has high sensitivity but low specificity for PTB, making it suitable for HIV+ patients. An algorithm combining CRP triage with Xpert MTB/Rif confirmatory testing can reduce the usage of Xpert while maintaining a high sensitivity. Further research is needed to evaluate the potential of CRP in prospective studies using point-of-care platforms.
