Diagnostic performance of morning serum cortisol as an alternative to short synacthen test for the assessment of adrenal reserve; a retrospective study

Summary: The study aimed to determine the relationship between morning serum cortisol levels and adrenal insufficiency (AI) in order to reduce the reliance on the short synacthen test (SST). The results indicated that a morning serum cortisol level of >= 354 nmol/L could predict normal adrenal function with 100% sensitivity and potentially avoid approximately 37% of SST tests.

Biological Effective Dose as a Predictor of Hypopituitarism After Single-Fraction Pituitary Adenoma Radiosurgery: Dosimetric Analysis and Cohort Study of Patients Treated Using Contemporary Techniques

Summary: This study found that biological effective dose (BED) is a more reliable predictor of post-SRS hypopituitarism compared to mean gland dose in patients with pituitary adenoma. Adjusted for sex and gland volume, BED > 45 Gy and mean gland dose > 10 Gy were significantly associated with an increased risk of hypopituitarism. Additional studies are needed to confirm these results.

Summary: Traumatic brain injury (TBI)-related hypopituitarism has been found to be more common than previously thought, affecting pituitary hormones such as growth hormone, gonadotropins, thyroid hormones, etc. The mechanisms of pituitary damage in TBI patients involve primary and secondary injuries, with long-standing growth hormone deficiency leading to neurocognitive and behavioral deficits. Further research is needed to better understand and characterize this clinical syndrome.
