Misdiagnosis of aortic dissection: A systematic review of the literature

Delayed sternal closure for intractable bleeding after acute type A aortic dissection repair: outcomes and risk factors analyses

Summary: Delayed sternal closure (DSC) is an efficient technique for stabilizing patients with intractable bleeding after acute type A aortic dissection (ATAAD) repair surgery. This retrospective study compared the outcomes and risk factors in patients who underwent DSC versus primary sternal closure (PSC) during ATAAD repair surgery. The study found that the DSC group had higher rates of hemopericardium and preoperative malperfusion, as well as longer cardiopulmonary bypass and aortic clamping times and a higher rate of intraoperative extracorporeal membrane oxygenation (ECMO) support. However, there were no significant differences in in-hospital mortality rates, 5-year survival rates, and freedom from reoperation rates between the DSC and PSC groups. Hemopericardium, preoperative malperfusion, and intraoperative ECMO support were identified as risk factors for implementing DSC.
