Metabolic Syndrome: Updates on Pathophysiology and Management in 2021

Gracia Fahed et al.

Summary: Metabolic syndrome is a cluster of metabolic dysregulations associated with insulin resistance and chronic low-grade inflammation. If left untreated, it significantly increases the risk of developing diabetes and cardiovascular diseases. However, many aspects of Metabolic syndrome are still not completely understood, and many questions remain unanswered. This review provides a comprehensive overview of the historical background, epidemiology, histopathology, pathophysiology, management, and prevention of Metabolic syndrome.

Metabolic Syndrome Pathophysiology and Predisposing Factors

Antonio Bovolini et al.

Summary: Metabolic syndrome is a cluster of cardiometabolic risk factors with high prevalence among adult populations and elevated costs for public health systems worldwide. It is characterized by the coexistence of risk factors such as abdominal obesity, atherogenic dyslipidemia, elevated blood pressure, a prothrombotic and pro-inflammatory state, insulin resistance, and higher glucose levels. These factors are indubitably linked to an increased risk of developing chronic conditions like type 2 diabetes and cardiovascular disease.

Role of Vitamin D in the Metabolic Syndrome

Lucia Melguizo-Rodriguez et al.

Summary: The prevalence of hypovitaminosis D has been increasing in developed countries due to lifestyle changes and unhealthy habits. Scientific evidence shows an inverse relationship between vitamin D deficiency and metabolic syndrome (MetS), with supplementation helping improve outcomes in individuals with MetS. Maintaining optimal vitamin D concentrations is proposed as a preventive strategy against MetS.

Effect of Vitamin D Supplementation on Glycemic Control in Prediabetes: A Meta-Analysis

Yujing Zhang et al.

Summary: The clinical research results on vitamin D supplementation in improving prediabetes are controversial. However, this meta-analysis found that supplementing with vitamin D led to significant improvements in FBG, HbA1c, and FINS among prediabetics. Subgroup analysis showed that certain populations, such as those in Asia and individuals with vitamin D deficiency, may benefit more from long-term vitamin D supplementation in terms of glucose metabolism and insulin resistance.

The Effects of Vitamin D Supplementation on Metabolic and Oxidative Stress Markers in Patients With Type 2 Diabetes: A 6-Month Follow Up Randomized Controlled Study

Milena Cojic et al.

Summary: The study suggests that daily oral doses of vitamin D can improve HbA1c levels over a period of 3 and 6 months, with a significant decrease in advanced oxidation protein products levels after 3 months of higher vitamin D doses. However, the impact of vitamin D on the HOMA-IR index, malondialdehyde levels, and TG/TBARS index was not statistically significant. Further research should focus on determining the appropriate doses of vitamin D in patients with type 2 diabetes to mitigate oxidative stress risk, metabolic syndrome risk, and related cardiovascular events.

Global View of Per Capita Daily Vitamin D Supply Estimates as Proxy Measures for Vitamin D Intake Data

Kevin D. Cashman

Summary: Nationally representative data on vitamin D intake can inform on the adequacy of dietary supply of vitamin D in a population, but such data is lacking for a majority of countries. Estimates of average per capita supply of vitamin D, as calculated using information from the Food and Agriculture Organisation of the United Nations (FAO) national food balance sheets (FBSs), were used to generate such average per capita vitamin D supply estimates for 173 to 178 countries around the globe. Fortification of milk and dairy or wheat flour with vitamin D had an important impact on the vitamin D supply estimates.

Summary: Vitamin D supplementation significantly improves hormone, inflammation, and oxidative damage levels in PCOS patients, but the effects on different parameters vary. Co-supplementation, low-dose vitamin D (<=1000 IU/day), and daily supplementation frequency appear to be more beneficial for improving the condition in PCOS patients.
