Total coumarin derivates from Hydrangea paniculata attenuate renal injuries in cationized-BSA induced membranous nephropathy by inhibiting complement activation and interleukin 10-mediated interstitial fibrosis

Summary: The study evaluated the renal protective effect of Hydrangea Paniculata, Sieb (HP) in an experimental model of membranous nephritis (MN) and explored its underlying mechanisms. HP administration improved renal function, reduced dyslipidemia and histopathological injuries. mRNA sequencing indicated anti-inflammation and anti-fibrosis effects of HP, possibly through down-regulating the complement activation and PI3K-AKT pathways. Pharmacokinetic studies identified skimmin and 7-hydoxycoumarin (7-HC) as major compounds in HP. Inflammatory macrophage IL10 production was inhibited by skimmin and 7-HC through blocking PI3K-AKT and NF Kappa B signaling pathways. HP protected tubulointerstitium from complement attack by reducing C3 self-production and auto-cleavage in tubular cells.

Efficacy and Safety of Different Immunosuppressive Therapies in Patients With Membranous Nephropathy and High PLA2R Antibody Titer

Summary: This study evaluated clinical features and prognosis of patients with seropositive M-type phospholipase-A2-receptor (PLA2R)-associated membranous nephropathy (MN) based on different risk ranks. The results showed that patients with high titer PLA2Rab had more severe proteinuria and hypoalbuminemia, lower remission rates, and longer time to remission compared to those with non-high titer antibody. The study also found that PLA2Rab level at baseline was an independent predictive factor for remission.
