Clinical manifestation and long-term outcome of citrin deficiency: Report from a nationwide study in Japan

Jun Kido et al.

Summary: This study investigates the clinical manifestations, medical therapy, and long-term outcome in 222 Japanese patients with citrin deficiency. The results show that patients with citrin deficiency have an increased risk for low weight and length at birth, and citrullinemia patients have an increased risk for growth impairment during adolescence. Liver transplantation has shown positive outcomes in a small number of patients.

Pathogenic variants of the mitochondrial aspartate/glutamate carrier causing citrin deficiency

Sotiria Tavoulari et al.

Summary: Citrin deficiency is a globally prevalent mitochondrial disease with three stages: neonatal intrahepatic cholestasis, a mild adaptation stage, and adult citrullinemia. It is caused by the absence or dysfunction of the calcium-regulated mitochondrial aspartate/glutamate carrier 2 (AGC2/SLC25A13), also known as citrin. This deficiency affects the malate-aspartate shuttle, gluconeogenesis, amino acid homeostasis, and the urea cycle.

L-Serine links metabolism with neurotransmission

Marianne Maugard et al.

Summary: Brain energy metabolism involves the intricate relationship between neurons and glial cells, with a specific metabolic pathway connecting glial metabolism to synaptic activity and plasticity through L-serine biosynthesis. Modulating astrocyte-mediated L-serine homeostasis may offer novel therapeutic approaches for brain disorders.

The utility of next-generation sequencing technologies in diagnosis of Mendelian mitochondrial diseases and reflections on clinical spectrum

Summary: The diagnostic process of mitochondrial disorders (MD) is challenging due to clinical variability and genetic heterogeneity. Next-Generation Sequencing (NGS) technology is a high-throughput platform for nuclear MD. Using WES and TES, new pathogenic gene variants were identified in MD, with WES being the primary method of choice for diagnosis.

Inborn disorders of the malate aspartate shuttle

Melissa H. Broeks et al.

Summary: The malate aspartate shuttle (MAS) is crucial for mitochondrial respiration and maintaining cytosolic redox balance, playing a key role in sustaining metabolic pathways like glycolysis and serine biosynthesis. Five potential MAS deficiencies have been reported, mostly presenting with infantile epileptic encephalopathy. Biochemical characteristics such as high lactate, disturbed redox balance, and low serine can aid in diagnosis, while treatment options include a ketogenic diet, serine, and vitamin B6 supplementation.

Triheptanoin-Novel therapeutic approach for the ultra-rare disease mitochondrial malate dehydrogenase deficiency

Alexander Laemmle et al.

Summary: Mitochondrial malate dehydrogenase deficiency (MDH2D) is an ultra-rare disease causing early onset encephalopathy with no curative therapy available. A female patient diagnosed with MDH2D through whole exome sequencing showed improvement in neurological and biochemical phenotype after undergoing a triheptanoin drug trial. Further research is needed to evaluate triheptanoin treatment for MDH2D and other related metabolic disorders.

Summary: The research updated the knowledge on treatable inherited metabolic disorders and developed a digital tool to assist in early recognition and intervention of these diseases. Through literature review, it was found that the number of treatable IDs increased by more than a third, with the majority of effective treatments being nutritional, showing significant efficacy.

Case Report and Review of the Literature: A New and a Recurrent Variant in the VARS2 Gene Are Associated With Isolated Lethal Hypertrophic Cardiomyopathy, Hyperlactatemia, and Pulmonary Hypertension in Early Infancy

Katarina Kusikova et al.

Summary: Mitochondriopathies encompass a wide range of genetic disorders affecting multiple systems, making diagnosis and treatment challenging. This study presents a male patient with mutations in the VARS2 gene, showing clinical symptoms such as hyperlactatemia, hypertrophic cardiomyopathy, and pulmonary hypertension leading to early death. These findings suggest the importance of considering mitochondriopathy and analyzing the VARS2 gene in cases of early onset and rapidly progressing HCM with hyperlactatemia.

Ketogenic diet for mitochondrial disease: a systematic review on efficacy and safety

Heidi Zweers et al.

Summary: The data on the efficacy and safety of ketogenic diet for mitochondrial disease is limited, making it difficult to make general recommendations. Ketogenic diet should be considered in individuals with mitochondrial disease and therapy-refractory epilepsy, but is contraindicated in mitochondrial DNA deletion-related myopathy. When considering ketogenic diet for mitochondrial disease, the high rate of adverse effects should be taken into account, as well as the spectacular improvements seen in individual cases.
