Clinical Study and Systematic Review of Pituitary Microadenomas vs. Macroadenomas in Cushing's Disease: Does Size Matter?

Amit Akirov et al.

Summary: This study compared the clinical and biochemical characteristics of patients with corticotroph microadenomas and macroadenomas. It was found that macroadenomas had higher levels of ACTH but no association was found between tumor size and cortisol hypersecretion or clinical features of hypercortisolemia. Furthermore, patients with macroadenomas with sphenoid or cavernous sinus invasion had higher biochemical burden, highlighting that extent/location of the adenoma may be more important than size per se.

Differences in outcomes of bilateral adrenalectomy in patients with ectopic ACTH producing tumor of known and unknown origin

Thomas Szabo Yamashita et al.

Summary: This study compared 48 patients undergoing BA for CS secondary to EK vs EU, with similar results in serum cortisol and ACTH concentrations. There was a higher percentage of EU BA performed minimally invasively, but it was not statistically significant. Complications, long term survival, and all-cause mortality did not show significant differences between EU and EK.

New types of localization methods for adrenocorticotropic hormone-dependent Cushing's syndrome

Russell Senanayake et al.

Summary: The management of endogenous Cushing's syndrome involves confirming autonomous hypercortisolism and localizing the cause, typically a pituitary corticotrope tumor or an ectopic ACTH syndrome due to a neuroendocrine tumor. Advanced imaging techniques such as MRI with higher field strength and molecular imaging like PET are becoming increasingly important in accurately identifying small tumors that may not be visualized on standard MRI. Specialized centers with a range of imaging techniques are ideal for managing ACTH-dependent CS effectively.

Case Report and Literature Review: Ectopic Thyrotropin-Secreting Pituitary Adenoma in the Suprasellar Region

Xiaoxu Li et al.

Summary: Ectopic thyrotropin-secreting pituitary adenoma (ectopic TSH-oma) is a rare disorder that is easily misdiagnosed in clinical work, mostly affecting middle-aged individuals but also seen in children. Achieving a timely and accurate diagnosis is challenging, requiring a combination of imaging examinations and special tests, with surgical resection leading to a satisfactory long-term outcome.

Invasive Corridor of Clivus Extension in Pituitary Adenoma: Bony Anatomic Consideration, Surgical Outcome and Technical Nuances

Xiao Wu et al.

Summary: This study investigated the bony anatomy associated with pituitary adenoma (PA) with clival invasion and its clinical significance. Through anatomical research and clinical case review, it was found that the petrous apex invasion of PA is caused by tumor crossing the petroclival fissure along the cancellous bone corridor. The study provides a deeper understanding of the invasive characteristics of PA.
