Aggressive fluid hydration plus non-steroidal anti-inflammatory drugs versus non-steroidal anti-inflammatory drugs alone for post-endoscopic retrograde cholangiopancreatography pancreatitis (FLUYT): a multicentre, open-label, randomised, controlled trial

Summary: This study aimed to evaluate the added value of aggressive periprocedural hydration in patients receiving prophylactic rectal NSAIDs. The results showed that aggressive hydration did not reduce the incidence of post-ERCP pancreatitis in patients with moderate to high risk of developing this complication who routinely received prophylactic rectal NSAIDs.

Comparing diagnostic accuracy of current practice guidelines in predicting choledocholithiasis: outcomes from a large healthcare system comprising both academic and community settings

Aswathi Chandran et al.

Summary: The 2019 ASGE guidelines are more specific for detecting choledocholithiasis during ERCP compared to the 2010 guidelines. However, a large number of patients are classified as intermediate-risk according to the 2019 guidelines and will require additional confirmatory imaging studies.

Evaluating the Revised American Society for Gastrointestinal . Endoscopy Guidelines for Common Bile Duct Stone Diagnosis

Jake S. Jacob et al.

Summary: The 2019 revision of the ASGE guidelines results in a decrease in the utilization of ERCP as a diagnostic modality and provides an improved risk stratification tool. The revised guidelines have higher specificity and potential to decrease the need for diagnostic ERCPs, with better identification of high-risk patients for choledocholithiasis. Specificity in diagnosing choledocholithiasis is increased with stone visualization on imaging, and gallstone pancreatitis does not increase the risk for choledocholithiasis.
