Timing of Kidney Support Therapy in Acute Kidney Injury: What Are We Waiting For?

Josee Bouchard et al.

Summary: The optimal timing of kidney support therapy in critically ill patients with acute kidney injury (AKI) without life-threatening complications related to AKI is controversial. This article reviews the concept of timing of dialysis in AKI, performs a critical assessment of the most important clinical trials in this topic, discusses ongoing research and knowledge gaps, and defines key research issues to address in the future.

Serum creatine kinase levels are not associated with an increased need for continuous renal replacement therapy in patients with acute kidney injury following rhabdomyolysis

Luniu Xiao et al.

Summary: This study investigated whether serum creatine kinase (CK) levels can serve as a marker for discontinuing continuous renal replacement therapy (CRRT) in patients with acute kidney injury (AKI) following rhabdomyolysis. The clinical outcomes were compared between patients with different CK levels after CRRT termination. The study found that higher CK levels and worse renal functions predicted the necessity of CRRT. After CRRT termination, the experimental group with CK > 5,000 U/L showed a significantly shorter in-hospital length of stay and ICU length of stay compared to the control group with CK < 5,000 U/L. These findings suggest that CRRT termination may be independent of CK levels in rhabdomyolysis-associated AKI patients, depending on their renal function recovery.

Complement activation is a crucial driver of acute kidney injury in rhabdomyolysis

Idris Boudhabhay et al.

Summary: The study revealed that both the lectin complement pathway and the alternative pathway are activated in rhabdomyolysis-induced acute kidney injury, leading to complement deposition and kidney injury. Experimental evidence showed the direct pathological role of complement in this process.

Development and validation of a model for the early prediction of the RRT requirement in patients with rhabdomyolysis

Summary: A model was developed and validated to predict the early requirement of RRT among RM patients based on 8 common variables measured within the first 24 hours after admission. Predicting the need for RRT could help ensure appropriate treatment and optimize the use of medical resources.

Summary: Patients with rhabdomyolysis (RM) following exertional heatstroke (EHS) often experience coagulation dysfunction and acute kidney injury (AKI). D-dimer is associated with AKI in these patients, serving as an independent risk factor for the development of AKI in RM patients.

Optimising the timing of renal replacement therapy in acute kidney injury

Matthew E. Cove et al.

Summary: The optimal timing of renal replacement therapy (RRT) in critically ill patients with acute kidney injury (AKI) has been debated, with high-quality evidence now supporting a deferred approach for up to 72 hours unless life-threatening indications develop. Physicians' judgment is still essential in identifying appropriate patients for expectant management.
