Severe Impairment of Microcirculatory Perfused Vessel Density Is Associated With Postoperative Lactate and Acute Organ Injury After Cardiac Surgery

John C. Greenwood et al.

Summary: Resuscitation after cardiac surgery must address multiple pathophysiological processes, including microcirculatory dysfunction which may lead to increased lactate levels and acute organ injury. A study on 25 adults undergoing elective cardiac surgery found that severely impaired perfused vessel density was significantly related to hyperlactatemia and early organ injury.

Increased cerebral mitochondrial dysfunction and reactive oxygen species with cardiopulmonary bypass

Lindsay E. Volk et al.

Summary: The study found that using cardiopulmonary bypass during cardiac surgery can lead to decreased cerebral mitochondrial respiration even in the absence of local markers of ischemia. Exposure to 4 hours of cardiopulmonary bypass resulted in a significant increase in cerebral mitochondrial reactive oxygen species formation.

Low Microcirculatory Perfused Vessel Density and High Heterogeneity are Associated With Increased Intensity and Duration of Lactic Acidosis After Cardiac Surgery with Cardiopulmonary Bypass

John C. Greenwood et al.

Summary: This study found that low perfused vessel density and high microcirculatory heterogeneity are associated with an increased intensity and duration of lactic acidosis after cardiac surgery with cardiopulmonary bypass.

Effect of Vitamin C, Thiamine, and Hydrocortisone on Ventilator- and Vasopressor-Free Days in Patients With Sepsis The VICTAS Randomized Clinical Trial

Jonathan E. Sevransky et al.

Summary: A trial on the effects of a combination treatment of vitamin C, thiamine, and hydrocortisone on ventilator- and vasopressor-free days in patients with sepsis did not show a significant increase within 30 days. However, the trial was terminated early and may have been underpowered to detect a clinically important difference.
