Prolonged Continuous Monitoring of Regional Lung Function in Infants with Respiratory Failure

Tobias H. Becher et al.

Summary: This study evaluated the feasibility and safety of long-term monitoring using electrical impedance tomography (EIT) in neonates and young children with respiratory failure. The results showed that EIT allowed continuous monitoring of lung function for up to 72 hours with minimal adverse effects. The study also found that ventilation distribution was influenced by body position and varied depending on the postmenstrual age of the patients.

Viral etiology and outcome of severe lower respiratory tract infections among critically ill children admitted to the PICU

M. Duyu et al.

Summary: This study conducted in the Pediatric Intensive Care Unit of Istanbul Medeniyet University in Turkey found that RSV virus was the most common cause of severe lower respiratory tract infections in critically ill children, with bronchiolitis being the most common diagnosis. Complications such as shock, acute respiratory distress syndrome (ARDS), hepatitis, and multiple viral infections were associated with the need for invasive mechanical ventilation in severely ill children.

Electrical impedance tomography in pediatric patients with COVID-19, the first reports

Milena S. Nascimento et al.

Summary: This study evaluated the ventilation distribution in pediatric COVID-19 patients using EIT and compared it to standard radiological imaging. The findings suggest that EIT is a promising tool for investigating ventilatory profiles in different lung diseases, including COVID-19, and may help reduce the need for repeated ionizing radiation exams in children.

Early Use of Bronchodilators and Outcomes in Bronchiolitis

Kristen H. Shanahan et al.

Summary: According to the recommendation of the American Academy of Pediatrics, routine use of bronchodilators for infants with bronchiolitis does not show significant effects. From 2010 to 2018, the use of bronchodilators, hospital admissions, and emergency department visits decreased, while ICU admissions and respiratory support increased.

For which infants with viral bronchiolitis could it be deemed appropriate to use albuterol, at least on a therapeutic trial basis?

Carlos E. Rodriguez-Martinez et al.

Summary: The study suggests that in older infants, those with rhinovirus bronchiolitis, a nasopharyngeal microbiome dominated by Haemophilus influenzae, affected during nonpeak or non-RSV predominant months, wheezing at presentation, clinical characteristics like atopic dermatitis or a family history of asthma, and infants infected with RSV genotypes ON1 and BA are most likely to benefit from albuterol. These findings could serve as a basis for rational albuterol administration in viral bronchiolitis patients and guide future clinical trials.
