Summary: This study found that (epi)genetic predisposing factors were present in at least 33.3% of pediatric patients with Wilms tumor in the Netherlands. Multiple predisposing genes associated with WT were identified, but further validation is needed.

TRIM28 variants and Wilms' tumour predisposition

Janna A. Hol et al.

Summary: TRIM28 has been identified as a predisposition gene for Wilms' tumour (WT), with germline pathogenic variants associated with epithelial WT. Loss of TRIM28 protein expression in tumour tissue is an effective strategy for identifying patients with TRIM28 variants. Further exploration of TRIM28-associated WT will help uncover the mechanisms underlying tumour development.

Bilateral Nephroblastic Tumors and a Complex Renal Vascular Anomaly in a Patient With a Mosaic RASopathy: Novel Histopathologic Features and Molecular Insights

Jonathan C. Slack et al.

Summary: Mosaic RASopathies are a group of emerging disorders characterized by activating mutations in genes of the RAS/MAPKinase signaling pathway. The phenotype is highly variable and can include malignancies. This report presents a case of somatic RASopathy in a boy, demonstrating diverse phenotypic features and potential pathogenic overlap. The occurrence of a secondary cancer hotspot mutation in the Wilms tumor suggests involvement of additional driver mutations in tumor development in somatic overgrowth disorders.

Summary: Beckwith-Wiedemann Spectrum (BWSp) is a common epigenetic childhood cancer predisposition disorder, with tumor risk and phenotypic differences compared to Classic BWS. Certain phenotypic features may be associated with specific epigenetic causes, indicating a need for further research.

Novel findings and expansion of phenotype in a mosaic RASopathy caused by somatic KRAS variants

Caitlin A. Chang et al.

Summary: This study describes eight individuals with somatic KRAS variants and associated features, including overgrowth and vascular or lymphatic malformations. The study expands the association with embryonal tumors, reports rare or novel findings, and describes the first use of targeted therapy with a MEK inhibitor for an individual with a mosaic KRAS variant. The purposes of the report are to expand the phenotypic spectrum of mosaic KRAS-related disorders and propose possible mechanisms of pathogenesis.

Janna A. Hol et al.

Summary: The updated Wilms tumour surveillance guidelines have been released for clinical geneticists, pediatricians, pediatric oncologists, and radiologists caring for children at risk of WT. These guidelines stress the importance of registering all patients with cancer predisposition syndromes in order to improve future tumor risk estimation and surveillance programs.

Summary: WAGR syndrome is a rare genetic condition with an increased risk of developing Wilms tumor. Patients with WAGR and Wilms tumor can be treated according to existing protocols, but monitoring of treatment complications and surveillance of the remaining kidney(s) are advised for optimal outcomes.
