Serum Based miRNA as a Diagnostic Biomarker for Multiple Sclerosis: a Systematic Review and Meta-Analysis

Summary: The systematic review and meta-analysis identified 19 serum miRNAs differentially expressed in multiple sclerosis patients, with 8 downregulated, 11 upregulated, and 1 discordant. The pooled AUC was 72% for overall MS patients and PPMS, indicating high sensitivity and specificity. A miRNA panel of four miRNAs showed high sensitivity (73%) and specificity (68%) in distinguishing MS from control groups.

Summary: In this study, the expression levels of hsa-let7d-5p and hsa-let7g-5p miRNAs were found to be significantly increased in patients with Alzheimer's disease (AD), while there was no significant difference in the expression of miR-15b-5p. These findings suggest the potential of measuring the transcript levels of hsa-let7d-5p and hsa-let7g-5p as diagnostic biomarkers for AD.

Current and Future Biomarkers in Multiple Sclerosis

Summary: Multiple sclerosis (MS) is a debilitating autoimmune disorder with a lack of effective treatment for the progressive form. Neurofilament light chain (NfL) has emerged as a potential biomarker for predicting MS disease activity and progression, but it is not specific to MS and can be influenced by age, BMI, and blood volume. Other biomarkers of axonal damage, neuronal damage, glial dysfunction, demyelination, and inflammation have also been studied, but they may face similar limitations as NfL. A comprehensive panel including cellular studies, miRNAs, EVs, metabolomics, metabolites, and the microbiome could provide a more useful approach. Further research using advanced technology and statistical approaches is needed to identify novel and useful biomarkers for MS.
