Poor Outcomes in Patients With Transplant Glomerulopathy Independent of Banff Categorization or Therapeutic Interventions

Summary: Transplant glomerulopathy (TG) can indicate different diseases including chronic antibody-mediated rejection (AMR). This study retrospectively investigated 282 kidney transplant recipients with biopsy-proven TG and diagnosed them according to the Banff'17 criteria. The long-term outcomes of allografts with AMR and TG were similar, and antihumoral therapy did not significantly affect the clinical outcomes. The Banff lesions indicating progressive scarring were better predictors of unfavorable outcomes.

The results showed that factors such as estimated GFR, proteinuria, time posttransplant, donor-specific antibody, as well as molecular and histologic features reflecting injury were more important predictors of failure compared to rejection activity.

The clinical and histological heterogeneity of ABMR is inadequately represented in a binary classification system.

Missing Self?Induced Microvascular Rejection of Kidney Allografts: A Population-Based Study

Summary: This study found that missing self increases the risk of microvascular inflammation after kidney transplantation, especially when multiple types of missing self occur simultaneously, independent of HLA-DSA. Systematic evaluation of missing self may improve understanding of HLA-DSA-negative MVI and could be relevant for improved diagnostic classification and patient risk stratification.
