Bilateral simultaneous non-arteritic ischaemic optic neuropathy: a rare complication of idiopathic systemic capillary leak syndrome (SCLS)

Summary: This case study presents a 72-year-old man who developed bilateral symmetrical superior visual field defects during recovery from SCLS. Despite extensive fluid replacement and mechanical ventilation during acute illness, his visual field defect due to ischaemic optic neuropathy remained stable over 3 years, with good visual acuity. The mechanism of optic nerve infarction in this case is suspected to be related to hypercoagulability, hypovolemia, vascular disease, and hypertension.
