How crises work: A model of error cause and effect in surgical practice

Summary: This study presents a crisis model in surgery to aid the reporting and management of safety critical events. The model suggests that surgical crises are triggered by four categories of threat and driven by only three types of error. These findings provide new opportunities for safety interventions and crisis management.

Crisis Preparedness: A Systems-Based Framework for Avoiding Harm in Surgery

Summary: This study aims to create a practical framework for crisis preparedness in surgery by identifying and translating crisis preparedness interventions from high-reliability industries to clinical practice. The study used observational fieldwork in commercial aviation, semi-structured interviews with senior airline pilots, and mixed focus groups with healthcare and aviation safety experts to derive a crisis preparedness framework.
