ALIF Versus TLIF for L5-S1 Isthmic Spondylolisthesis: ALIF Demonstrates Superior Segmental and Regional Radiographic Outcomes and Clinical Improvements Across More Patient-reported Outcome Measures Domains

Summary: This study compared the radiographic parameters and clinical outcomes of anterior interbody fusion (ALIF) and posterior interbody fusion (TLIF) for the treatment of isthmic spondylolisthesis. The results showed that ALIF generated greater segmental and regional lordosis, as well as better restoration of disc height compared to TLIF. ALIF patients also demonstrated significant improvements in a broader range of patient-reported outcome measures (PROMs) domains than TLIF patients.

Summary: Patients undergoing anterior lumbar interbody fusion (ALIF) showed significantly improved rates of minimum clinically important difference (MCID) achievement for disability, physical function, and back pain during the early postoperative period. However, the overall MCID achievement rate for leg pain was higher for the minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) cohort.

Summary: Indirect decompression via anterior lumbar interbody fusion (ALIF) is effective in treating selected patients with degenerative lumbar spinal stenosis, but older patients are at higher risk for failure. Patients in both ALIF and ALIF + posterior instrumentation subgroups showed significant improvement in symptoms pre- and post-operatively.
