BCLC strategy for prognosis prediction and treatment recommendation: The 2022 update

Maria Reig et al.

Summary: There have been significant advances in the treatment strategies for hepatocellular carcinoma since the last update in 2018. However, some interventions still lack sufficient data for clinical incorporation, and personalized clinical management requires consideration of multiple parameters.

Tumor Necrosis Impacts Prognosis of Patients Undergoing Curative-Intent Hepatocellular Carcinoma

Tao Wei et al.

Summary: The study found that tumor necrosis is associated with worse prognosis for HCC patients, and the extent of necrosis is correlated with the severity of tumor characteristics. Patients with more extensive necrosis have shorter overall survival.

Imaging features based on Gd-EOB-DTPA-enhanced MRI for predicting vessels encapsulating tumor clusters (VETC) in patients with hepatocellular carcinoma

Yanfen Fan et al.

Summary: The study investigated the non-invasive prediction of hepatocellular carcinoma (HCC) with vessels encapsulating tumor clusters (VETC) based on qualitative and quantitative imaging features of gadolinium-ethoxybenzyl-diethylenetriamine pentaacetic acid (Gd-EOB-DTPA)-enhanced MRI. It found that certain imaging features and laboratory examination results can accurately predict VETC status in HCC, providing potential guidance for patient outcomes and treatment decisions. The nomogram based on multivariate analysis showed good diagnostic performance with an area under the receiver operating characteristics curve value of 0.885.

Hepatocellular carcinoma

Josep M. Llovet et al.

Summary: Liver cancer, particularly hepatocellular carcinoma (HCC), poses a significant global health challenge with rising incidence projected to exceed 1 million cases by 2025. Infection by hepatitis B and C viruses remains a major risk factor for HCC, while non-alcoholic steatohepatitis is emerging as a more common risk factor in Western countries. Advances in systemic therapies for HCC, including immunotherapies and targeted therapies, are expected to revolutionize the management of this disease.

Global cancer statistics 2020: GLOBOCAN estimates of incidence and mortality worldwide for 36 cancers in 185 countries

Hyuna Sung et al.

Summary: The global cancer burden in 2020 saw an estimated 19.3 million new cancer cases and almost 10.0 million cancer deaths. Female breast cancer surpassed lung cancer as the most commonly diagnosed cancer, while lung cancer remained the leading cause of cancer death. These trends are expected to rise in 2040, with transitioning countries experiencing a larger increase compared to transitioned countries due to demographic changes and risk factors associated with globalization and a growing economy. Efforts to improve cancer prevention measures and provision of cancer care in transitioning countries will be crucial for global cancer control.

Pathologic, Molecular, and Prognostic Radiologic Features of Hepatocellular Carcinoma

Kathryn J. Fowler et al.

Summary: Hepatocellular carcinoma is a malignant tumor with varying aggressiveness based on tumor grade, vascular invasion, and pathologic molecular classification. Knowledge of different pathologic and molecular phenotypes of HCC is important for prognosis, and imaging plays a central role in diagnosis.

Preoperative CT for Characterization of Aggressive Macrotrabecular-Massive Subtype and Vessels That Encapsulate Tumor Clusters Pattern in Hepatocellular Carcinoma

Zhichao Feng et al.

Summary: This study aimed to evaluate the potential of preoperative CT in predicting the MTM subtype and VETC pattern of HCC. Specific CT imaging features were identified as predictors for tumor subtype, leading to the development of prognostic scoring systems. The predictive performance was validated in a separate cohort, with high ANH and high SN phenotype being independently associated with early recurrence after surgical resection.

Prognostic Impact of Vessels that Encapsulate Tumor Cluster (VETC) in Patients who Underwent Liver Transplantation for Hepatocellular Carcinoma

Junji Kawasaki et al.

Summary: VETC expression in HCC is strongly correlated with malignant potential and overall survival after LDLT. VETC(+) patients have lower CD3(+) cell ratios, indicating a relationship between VETC activity and lymphocyte activity. Combination of VETC expression with low CD3 levels is an independent risk factor for mortality.

Gadoxetate-enhanced MRI Features of Proliferative Hepatocellular Carcinoma Are Prognostic after Surgery

Hyo-Jin Kang et al.

Summary: This study evaluated the gadoxetate-enhanced MRI findings and prognostic significance of the proliferative class of hepatocellular carcinoma (HCC) after surgery. The proliferative class was associated with poor overall survival, increased rates of intrahepatic and extrahepatic metastasis, and specific predictive factors such as rim arterial phase hyperenhancement on MRI and high serum alpha-fetoprotein levels.

Roberto Cannella et al.

Summary: In this study, the distribution of LI-RADS major features and categories did not differ among different histological subtypes of hepatocellular carcinoma in high-risk patients. However, tumour-in-vein, LR-M features, and ancillary features can provide valuable information for the non-invasive diagnosis of hepatocellular carcinoma subtypes.
