Summary: This study aims to investigate the frequency and outcomes of aortic valve reintervention after transcatheter aortic valve replacement, and found that patients who underwent transcatheter aortic valve explantation had a higher risk of mortality.

Coronary Access After Transcatheter Aortic Valve Replacement With Commissural Alignment: The ALIGN-ACCESS Study

Giuseppe Tarantini et al.

Summary: This study investigated the impact of commissural alignment on the feasibility of coronary access after transcatheter aortic valve replacement (TAVR). The results showed that commissural alignment improves the rate of selective coronary access, but aligned supra-annular transcatheter heart valves (THVs) have a higher risk of unfeasible/nonselective coronary access compared to SAPIEN 3 THVs.

Summary: This study aimed to determine the impact of different implant depths of the Sapien 3 transcatheter heart valve within the Evolut THV on performance and function. Results showed that lower implant positions do not significantly compromise valve function and may facilitate future coronary access.

Anatomical Predictors of Pacemaker Dependency After Transcatheter Aortic Valve Replacement

Summary: Less than half of TAVR patients needing pacemaker implantation are pacemaker-dependent at midterm follow-up. Factors predicting long-term pacemaker dependency after TAVR include left ventricular outflow tract calcifications under the left coronary cusp and ΔMSID >= 3 mm.

Summary: This study compared the efficacy of TAVR in failed transcatheter aortic valves (TAVs) versus surgical aortic valves (SAVs) and found that TAVR in TAV failures had higher procedural success with similar procedural safety and mortality compared to TAVR in SAV.

Summary: The 1-year outcomes of the BASILICA trial showed no late strokes, myocardial infarctions, or deaths related to the procedure, with the mitigation of coronary artery obstruction remaining intact at 1 year. These results provide reassurance for patients and physicians looking to avoid long-term complications associated with snorkel stenting.

Summary: The frequency of TAVR-explant surgery is low but increasing, with an operative mortality rate of 20% and high rate of in-hospital complications. Common indications for surgery include procedure-related failure, paravalvular leak, and the need for other cardiac surgery.

Valve Academic Research Consortium 3: Updated Endpoint Definitions for Aortic Valve Clinical Research

Philippe Genereux et al.

Summary: The Valve Academic Research Consortium (VARC) was established in 2010 to standardize clinical endpoint definitions for aortic valve clinical trials. After several years, a meeting was held involving experts to evaluate and revise specific clinical endpoint definitions, leading to the proposal of new classifications and modifications. These updates will ensure accurate reporting and comparisons in clinical research studies involving aortic valve therapies.

Follow-up at one year showed relatively low mortality and hospitalization rates.
