High 27-OH levels may alter synaptic potentiation and disrupt fine-tuned processing of information in hippocampal circuits, potentially causing cognitive impairment.

27-Hydroxycholesterol, cognition, and brain imaging markers in the FINGER randomized controlled trial

Summary: The reduction of 27-hydroxycholesterol (27-OH) during intervention was associated with improved cognition, especially memory. The intervention particularly reduced 27-OH levels in individuals with higher levels and younger age. However, no associations were found between change in 27-OH levels and neuroimaging markers. At baseline, higher 27-OH levels were associated with lower gray matter and hippocampal volume, as well as lower cognitive scores, which were unaffected by total cholesterol levels.
