Summary: Hypertrophic cardiomyopathy (HCM), a common and often inherited primary cardiac disease, can be effectively treated with various options that significantly improve patient outcomes. Innovations in risk stratification, surgical intervention, anticoagulation prophylaxis, and ablation techniques have reduced morbidity and mortality rates. Palliative pharmacological strategies can control symptoms in the short-term, but have limited impact on long-term clinical course.

Global longitudinal diastolic strain rate as a novel marker for predicting adverse outcomes in hypertrophic cardiomyopathy by cardiac magnetic resonance tissue tracking

Summary: This study examined the prognostic value of global peak diastolic strain rate (PDSR) derived from cardiac magnetic resonance tissue tracking in predicting adverse outcomes in hypertrophic cardiomyopathy (HCM) patients. The results showed that longitudinal PDSR remained the strongest predictor of outcome, demonstrating potential for risk stratification and benefit to patient management. Further confirmatory studies are warranted to validate these findings.

Summary: This study evaluated the utility of myocardial strain for detecting LVOT obstruction in HCM patients based on magnetic resonance tissue tracking. The combination of Maximum LVWT >22.9 mm and Sub Radial PS >8.1% showed promise for objectively evaluating LVOT obstruction with very high specificity and acceptable sensitivity.

Prognostic value of myocardial extracellular volume fraction evaluation based on cardiac magnetic resonance T1 mapping with T1 long and short in hypertrophic cardiomyopathy

Summary: In patients with hypertrophic cardiomyopathy (HCM), extracellular volume (ECV) is a strong imaging marker for predicting adverse outcomes. ECV has a strong correlation with left ventricular ejection fraction (LVEF) and left ventricular end-diastolic volume index (LVEDVI), and can evaluate myocardial tissue structure and function. ECV has stronger predictive effectiveness than late gadolinium enhancement (LGE), and even in the subgroup with LGE, ECV shows independent predictive significance for adverse events.

Summary: Assessment of left ventricular function is crucial in decision-making, and myocardial deformation imaging using speckle-tracking has become a feasible clinical tool with greater effectiveness in measuring strain compared to strain-rate. Strain measurements have clinical utility in subclinical LV dysfunction, RV dysfunction, atrial fibrillation, and sequential follow-up.

Summary: The study found that global longitudinal strain is a strong independent predictor for predicting the development of progressive heart failure symptoms and/or systolic dysfunction in patients with nonobstructive hypertrophic cardiomyopathy. Combining global longitudinal strain with ejection fraction to assess the cardiac burden is an effective way to predict heart failure progression and systolic dysfunction in nonobstructive hypertrophic cardiomyopathy patients.
