H. pylori-positive samples exhibit reduced microbial diversity, altered microbiota community, and weaker interactions among gastric microbes.

Efforts to improve cancer prevention measures and provision of cancer care in transitioning countries will be crucial for global cancer control.

Summary: Microbial infections are known to contribute to gastric carcinogenesis, and changes in gastric microbiota may aid in early diagnosis of gastric cancer. This study aimed to characterize microbial changes and identify taxonomic biomarkers across stages of gastric carcinogenesis. The results showed that the richness and diversity of gastric microbiota did not show a gradual changing trend, with certain oral bacteria continuously enriching from superficial gastritis to gastric cancer. Patients with gastric cancer had enrichments of both oral and environmental bacteria, while those with gastric intraepithelial neoplasia had an enrichment of intestinal commensals. Gastric cardia cancer and non-cardia cancer patients had significantly different microbiota profiles, with a higher abundance of Helicobacter in the cardia cancer patients.

Heparanase sustains a vicious cycle of inflammation and tumor progression by activating macrophages and promoting cytokine release, potentially increasing the risk of gastric cancer. Inhibition of heparanase may be a potential therapeutic strategy for reducing the risk of gastritis and gastric cancer.
