Adult complications of nephropathic cystinosis: a systematic review

Rachel Nora Kasimer et al.

Summary: Nephropathic cystinosis, traditionally seen as a childhood disease, is now allowing patients to live into adulthood with improved treatments. Complications faced by aging patients with cystinosis affect almost every organ system, with common adult-onset issues including myopathy, diabetes, and hypothyroidism. Further research is needed to explore the effects of cysteamine on nonrenal manifestations of cystinosis and to develop new treatments.

Nephropathic cystinosis: an update on genetic conditioning

Rezan Topaloglu

Summary: Cystinosis is an autosomal recessive lysosomal storage disorder caused by CTNS gene mutations. Treatment with cysteamine can slow organ deterioration, but is ineffective against renal Fanconi syndrome and kidney failure. Researchers are interested in novel treatment modalities for this hereditary disease.

Drug Repurposing in Rare Diseases: An Integrative Study of Drug Screening and Transcriptomic Analysis in Nephropathic Cystinosis

Francesco Bellomo et al.

Summary: The article discusses a drug repurposing strategy for nephropathic cystinosis, a rare inherited disorder, using mechanism-based and cell-based screenings coupled with computational analysis to predict therapeutic responses. Through comparing gene-expression signatures of drugs and the disease, potential drugs and metabolic pathways relevant to the pathophysiology were identified.

Benefits and Toxicity of Disulfiram in Preclinical Models of Nephropathic Cystinosis

Anna Taranta et al.

Summary: Nephropathic cystinosis is a rare disease caused by mutations of the CTNS gene, leading to cystine accumulation in tissues. Treatment with DSF shows partial reversal of the disease, but high dosage inhibits growth, while low dosage fails to reduce cystine accumulation and causes adverse effects.

Review Molecular Mechanisms and Treatment Options of Nephropathic Cystinosis

Amer Jamalpoor et al.

Summary: Nephropathic cystinosis is a severe systemic disorder caused by mutations in the CTNS gene, primarily affecting the kidneys. Recent studies have shown that the loss of cystinosin leads to disrupted autophagy dynamics, accumulation of distorted mitochondria, and increased oxidative stress, resulting in abnormal proliferation and dysfunction of kidney cells.

A novel sustained-release cysteamine bitartrate formulation for the treatment of cystinosis: Pharmacokinetics and safety in healthy male volunteers

Cecile L. Berends et al.

Summary: The study investigated the pharmacokinetics, safety, and tolerability of a new sustained-release cysteamine dosage form, PO-001, compared to immediate-release Cystagon(R) and delayed-release Procysbi(R) in healthy volunteers. PO-001 showed sustained-release characteristics with lower C-max and longer T-max, while being well tolerated with only mild adverse events reported. Population PK simulations indicated a favorable PK profile, supporting further development of PO-001 for the treatment of cystinosis patients.

Cysteamine-bicalutamide combination therapy corrects proximal tubule phenotype in cystinosis

Amer Jamalpoor et al.

Summary: Through omics analysis, researchers identified alpha-ketoglutarate as a potential metabolite linking cystinosin loss to autophagy, apoptosis, and kidney proximal tubule impairment in cystinosis. Combining this insight with a drug screen, they discovered that a bicalutamide-cysteamine combination treatment could correct the phenotype of cystinotic kidney proximal tubule cells.

Human and animal fertility studies in cystinosis reveal signs of obstructive azoospermia, an altered blood-testis barrier and a subtherapeutic effect of cysteamine in testis

Ahmed Reda et al.

Summary: The vast majority of cystinosis patients develop azoospermia, possibly due to epididymal dysfunction. Cysteamine treatment does not affect fertility, but has limited effects on cystine accumulation in the testis.

Summary: This study investigated a large cohort of 453 patients with nephropathic cystinosis born between 1964 and 2016 and followed in multiple countries. The findings support early diagnosis and optimization of cystine depletion therapy for improved outcomes in nephropathic cystinosis.
