Exercise-induced pulmonary hypertension in HFpEF and HFrEF: Different pathophysiologic mechanism behind similar functional impairment

Nicola Riccardo Pugliese et al.

Summary: Despite similar disease severity, HFpEF and HFrEF show different pathophysiological mechanisms. HFpEF is characterized by a worse LA-LV and RV-PA interaction than HFrEF, with more prevalent low-grade systemic inflammation. These features may have a role in exercise-induced pulmonary hypertension in HFpEF.

Characterization of hemodynamic and metabolic abnormalities in the heart failure spectrum the role of combined cardiopulmonary and exercise echocardiography stress test

Nicola R. Pugliese et al.

Summary: Heart failure is a complex clinical syndrome characterized by different etiologies and a broad spectrum of cardiac structural and functional abnormalities. Current guidelines classify heart failure based on left ventricular ejection fraction, distinguishing between reduced and preserved ejection fraction. Heart failure can progress from asymptomatic stages to clinically manifest syndrome, and the progression is associated with prognosis. However, the understanding of the steps and risk factors leading to heart failure progression is still lacking, especially in preserved ejection fraction heart failure. Precise hemodynamic and metabolic characterization may help identify different disease trajectories and risk factors, leading to specific treatment targets.

Association between right-sided cardiac function and ultrasound-based pulmonary congestion on acutely decompensated heart failure: findings from a pooled analysis of four cohort studies

Masatake Kobayashi et al.

Summary: In patients with acutely decompensated heart failure, impaired right ventricular function and right ventricular-pulmonary artery coupling were associated with severe pulmonary congestion on admission, and less resolution of pulmonary congestion during hospital stay. Worse prognosis related to residual pulmonary congestion was enhanced in patients with right ventricular dysfunction.

Summary: Risk stratification based on a multi-parametric evaluation, including biomarkers, cardiopulmonary exercise testing-exercise stress echocardiography, and lung ultrasound, can effectively predict adverse events in HFpEF patients and individuals at risk for developing HF, offering a more personalized treatment approach.

Predictors and prognosis of right ventricular function in pulmonary hypertension due to heart failure with reduced ejection fraction

Alexander Schmeisser et al.

Summary: This study aimed to explore the coupling ratio of right ventricular and pulmonary artery function in patients with pulmonary hypertension (PH) to predict overall survival. The results showed that an appropriate coupling ratio was associated with higher mid-term survival rates, while an unfavorable coupling ratio was linked to higher mortality rates.
