Summary: This case report describes the first instance of delayed tracheal perforation following total thyroidectomy in a patient with a history of neck radiotherapy. The patient presented with neck surgical emphysema, which was initially misdiagnosed as a seroma. Proper imaging and multidisciplinary management were crucial for successful treatment.
