Summary: This study introduces two novel electrode configurations: bipolar and transmural, aiming to improve the efficacy of VT treatment while reducing the overall number of required therapies and need for strong backup shocks. The transmural configuration is the most efficient, requiring less energy to achieve high efficacy, and both configurations show higher efficacy when applied to slow VTs compared to fast VTs.

Summary: The majority of data on tissue substrate for post myocardial infarction (MI) VT has been collected during hemodynamically tolerated VT, but this study aimed to characterize tissue at diastolic locations of VT with hemodynamic compromise (VT-HC) in a porcine model. The results show that non-transmural scar, mildly decreased tissue thickness, and steep gradients in tissue thickness are structural characteristics of the diastolic component of reentrant circuits in VT-HC.
