Surgery and prophylactic surgery in hereditary breast cancer

Monica Morrow

Summary: Women with hereditary breast cancer have an increased risk of developing second primary cancers in both the affected and unaffected breast. The level of risk is influenced by the specific gene mutation and the age at which the first breast cancer was diagnosed. These factors, along with life expectancy, should be taken into account when deciding on the appropriate surgical approach.

Adjuvant abemaciclib combined with endocrine therapy for high-risk early breast cancer: safety and patient-reported outcomes from the monarchE study

H. S. Rugo et al.

Summary: The combination of abemaciclib and endocrine therapy as adjuvant treatment for early breast cancer shows significant improvement in invasive disease-free survival. Safety analysis reveals a higher incidence of grade >=3 adverse events and serious adverse events in the abemaciclib-treated group, but most adverse events are reversible and manageable with comedications and dose modifications.

Oligometastasis in breast cancer-current status and treatment options from a radiation oncology perspective

Marc D. Piroth et al.

Summary: Evidence suggests that SBRT treatment in oligometastatic breast cancer shows positive effects on local tumor control and survival rates, but it remains unclear which patients benefit the most from this approach. Further studies are needed to prove the benefits of SBRT in oligometastatic breast cancer and to define appropriate selection criteria.

Adjuvant abemaciclib combined with endocrine therapy for high-risk early breast cancer: updated efficacy and Ki-67 analysis from the monarchE study

N. Harbeck et al.

Summary: Adjuvant abemaciclib combined with endocrine therapy significantly improved invasive disease-free survival in patients with early breast cancer, with an acceptable safety profile. Ki-67 index was prognostic but did not affect the benefits of abemaciclib. The robust treatment benefit of abemaciclib extended beyond the planned 2-year treatment period.

Radiotherapy Delivered before CDK4/6 Inhibitors Mediates Superior Therapeutic Effects in ER+ Breast Cancer

Giulia Petroni et al.

Summary: Preclinical findings suggest that delivering radiotherapy before CDK4/6 inhibitors can achieve superior antineoplastic effects in breast cancer models, questioning the design of clinical trials administering CDK4/6 inhibitors before radiotherapy in women with ER+ breast cancer.

A Population-Based Study of Genes Previously Implicated in Breast Cancer

Chunling Hu et al.

Summary: This study provides population-based estimates of the prevalence and risk of breast cancer associated with pathogenic variants in known cancer-predisposition genes. The results highlight the varying levels of risk associated with different genes, such as high risk for BRCA1 and BRCA2 and moderate risk for PALB2, as well as associations with specific subtypes of breast cancer for genes like RAD51C and RAD51D.

Breast Cancer Risk Genes - Association Analysis in More than 113,000 Women

Leila Dorling et al.

Summary: This study sequenced samples from 60,466 women with breast cancer and 53,461 controls using a panel of 34 putative susceptibility genes, identifying associations between protein-truncating variants in certain genes and breast cancer risk. The results provide important information for genetic counseling and define genes that are most clinically useful for predicting breast cancer risk.

Updated recommendations regarding the management of older patients with breast cancer: a joint paper from the European Society of Breast Cancer Specialists (EUSOMA) and the International Society of Geriatric Oncology (SIOG)

Laura Biganzoli et al.

Summary: Breast cancer in older adults is becoming more common and challenging to manage due to its heterogeneity and lack of specific evidence. An expert taskforce expanded and updated guidelines with endorsement from the European Cancer Organisation, including factors like chemotherapy toxicity prediction, cultural and social considerations, and genetic screening. Age should not be the sole factor in decision-making, with careful consideration of geriatric assessments, life expectancy, and patient preferences.

Omission of adjuvant radiotherapy for older adults with early-stage breast cancer particularly in the COVID era: A literature review (on the behalf of Italian Association of Radiotherapy and Clinical Oncology)

Isabella Palumbo et al.

Summary: This review evaluates the possibility of omitting radiation therapy in older adults with early-stage low risk breast cancer. Recent literature, particularly during the COVID-19 pandemic, suggests that in a selected group of patients, omission of adjuvant RT may be considered based on certain favorable prognostic factors. Adjuvant RT significantly impacts local recurrence but not distant metastasis or death, leading to considerations of alternative regimens such as hypofractionated or ultra-hypofractionated protocols in certain cases.
