Summary: This article presents the updated recommendations from the French society of oncological radiotherapy on hepatic tumors. Recent technological advancements have led to the development of focused liver radiation therapy. It is important to differentiate between primary and secondary tumors and consider them as alternatives to surgery or medical treatments. Factors such as tumor volume and its proximity to organs at risk determine the appropriate radiation technique. Breathing-related motions should also be taken into account. Strict dosimetric criteria must be followed, especially for non-tumoral liver tissue and hollow organs, when using hypofractionated high dose radiotherapy under stereotaxic conditions. Stereotactic body radiotherapy is often preferred over radiofrequency for primary or secondary tumors smaller than 5 cm. An adaptive conformal fractionated irradiation protocol may be recommended for hepatocellular carcinomas larger than 5 cm.
