Renal gluconeogenesis: an underestimated role of the kidney in systemic glucose metabolism

The kidney contributes significantly to systemic gluconeogenesis, with lactate being the main substrate. Impaired renal gluconeogenesis in acute and chronic kidney disease can lead to systemic metabolic disturbances. Recently, a new hypothesis suggests that uric acid may lead to a pro-inflammatory status, endothelial dysfunction, and excessive activation of the renin-angiotensin-aldosterone system. Hyperuricemia associated with oxidative stress is also linked to DNA damage, oxidations, inflammatory cytokine production, and cell apoptosis. Increasing evidence shows the association of hyperuricemia with chronic kidney disease, cardiovascular disease, and metabolic syndrome or diabetes.
