Summary: This study evaluated the performance of neurofilament light chain (NfL) testing in diagnosing anti-NMDA receptor encephalitis (NMDARe) and first episode of psychosis (FEP) caused by a psychiatric disease. The results showed that NfL levels were associated with disease severity in NMDARe but not with long-term outcome. FEP patients with NfL levels ≥15 pg/mL had a significantly higher chance of having NMDARe.

Summary: Research has found that the cycloid psychosis (CP) phenotype is a common clinical presentation of psychiatric symptoms in patients with anti-NMDA receptor encephalitis (NMDARE), characterized by acute psychotic episodes with a sudden onset and a fluctuating clinical pattern, including confusion, delusions, hallucinations, motility disturbances, and mood oscillations. Patients who display the CP phenotype during their first psychotic episode should undergo cerebrospinal fluid analysis to rule out a possible diagnosis of autoimmune encephalitis.

Autoimmune Global Amnesia as Manifestation of AMPAR Encephalitis and Neuropathologic Findings

Summary: This study reported an unusual clinical phenotype of AMPAR encephalitis in 3 patients, presenting with acute-to-subacute global amnesia without affecting cognitive performance. Neuropathologic analysis revealed unilateral hippocampal sclerosis with AMPAR immunoreactivity decrease. Two patients achieved remission after immunotherapy, while one patient with lung adenocarcinoma did not improve and died due to tumor progression. AMPAR antibodies may cause isolated hippocampal dysfunction without overt inflammatory changes.
