Minimally invasive anatomic liver resection: Results of a survey of world experts

Mamoru Morimoto et al.

Summary: Minimally invasive anatomic liver resection is still a challenging procedure, but there is a certain degree of consensus among expert liver surgeons. Most experts have over 11 years of experience with MILR and recommend techniques such as creating a demarcation line, indocyanine green negative staining method, and intraoperative ultrasound for identifying IPs.

Summary: This study investigated the association between preoperative BMI and achieving textbook outcomes (TO) in patients undergoing hepatectomy for hepatocellular carcinoma (HCC). The results showed that both low and high preoperative BMI were independently associated with a lower likelihood of achieving optimal TO after surgery.

Textbook outcome among voluntary donors undergoing major living donor hepatectomy

Abu Bakar Hafeez Bhatti et al.

Summary: The study aimed to assess the rate and associated factors of composite measure of outcome (TO) after major living donor hepatectomy (MLDH). Among the 1022 donors studied, approximately 70% achieved TO with no donor mortality. The study also found a correlation between the early donation era and failure to achieve TO.

Summary: This study retrospectively analyzed more than 500 patients who underwent pure laparoscopic donor hepatectomy (PLDH) at five centers in Korea, presenting outcomes of both donors and recipients. Although there were complications during surgery, overall, the application of this technique showed promising clinical outcomes. Further studies are required to consider PLDH as a new standard practice.

Textbook outcomes in hepatobiliary and pancreatic surgery

Diamantis Tsilimigras et al.

Summary: The concept of textbook outcome (TO) is gaining popularity in surgical research as a way to evaluate the quality and success of different surgical procedures, including hepatopancreatobiliary (HPB) operations. TO is made up of individual outcome parameters reflecting different care domains, and can be used to assess patient-level outcomes, hospital performance, and quality metrics. Efforts are needed to establish a universal definition of TO in HPB surgery for comparing outcomes and improving patient results.

Joal D. Beane et al.

Summary: This analysis found that in the past five years, minimally invasive hepatectomies have increased in North America, while operative time and length of stay have decreased. The rates of postoperative sepsis, bile leaks, and liver failure have also decreased. Optimal hepatic surgery has increased for both partial and all hepatectomies, with it being achieved more often in partial resections.
