Assessment of Paravalvular Leak Severity and Thrombogenic Potential in Transcatheter Bicuspid Aortic Valve Replacements Using Patient-Specific Computational Modeling

Salwa B. Anam et al.

Summary: This study utilized patient-specific computational modeling to assess post-TAVR complications in BAV patients, finding a significant decrease in post-implantation paravalvular leak and thrombogenic potential with the latest generation TAVR devices. This highlights the potential of simulations in pre-procedural planning for better clinical outcomes and device selection.

Visions of TAVR Future: Development and Optimization of a Second Generation Novel Polymeric TAVR

Brandon Kovarovic et al.

Summary: Polymeric transcatheter aortic valve (TAV) devices offer a breakthrough approach for treating aortic valve stenosis and demonstrate comparable performance to tissue-based valves. Optimization of the device design and functionality improves durability and reduces the risk of thrombosis. This second-generation polymeric valve has undergone extensive testing and shows promising results for future use.

Validating In Silico and In Vitro Patient-Specific Structural and Flow Models with Transcatheter Bicuspid Aortic Valve Replacement Procedure

Salwa B. Anam et al.

Summary: This study combines in silico and in vitro methods to create patient-specific models and 3D printed replicas of three BAV patients with different degrees of post-TAVR paravalvular leakage (PVL). Computational fluid dynamics simulations and in vitro flow studies were performed to assess the degrees of PVL in these models. The results demonstrate good agreement between the in silico and in vitro models in terms of stent deformation and flow parameters.

Summary: Leaflet durability and costs limit the use of TAVR to elderly patients in wealthy countries. However, a new BE-TAVR system has been developed that allows for easy deployment, avoids secondary procedures, and is suitable for younger patients and non-calcific AR, expanding the global reach of TAVR.

Summary: This study develops a computational modeling approach to simulate the calcification progression in bicuspid aortic valve (BAV) and its biomechanical response during disease progression. The study sheds light on the fluid-structure mechanism that drives calcific aortic valve disease (CAVD) progression in BAV patients, by analyzing flow patterns and shear stresses on the leaflets during the full cardiac cycle.
