Optimal Aspergillus fumigatus and Asp f 1 serum IgG cut-offs for the diagnosis of allergic bronchopulmonary aspergillosis

Summary: This study aimed to determine cut-off levels of IgG-binding crude Af extracts or recombinant Asp f 1 and compare their efficacy for ABPA diagnosis with Af-precipitating Abs. It was found that cut-offs for Af-IgG levels varied by age, while a single cut-off was set for Asp f 1-IgG. Although IgG testing by EIA showed good diagnostic performance, Af-precipitating Abs were found to be more applicable for diagnosing ABPA.

Summary: This study compared the diagnostic performance of various criteria for evaluating ABPA and found that optimal diagnostic performance could be achieved by modifying the existing ISHAM criteria, with increased cost. The ISHAM-ABPA Working Group criteria were only marginally better than the Patterson criteria in diagnosing ABPA among patients with asthma younger than 66 years.
