Psychological Distress Increases 10-Fold Among People with HIV in the First Year on ART in Nigeria-a Call for Integrated Mental Health Services

Summary: This study found that the risk of psychological distress increased 30-fold in people with HIV (PWH) in Nigeria during the first year of antiretroviral therapy (ART). Factors such as age, gender, disclosure status, and duration on ART were associated with psychological distress, while being married was associated with reduced odds. Psychological distress at 12 months was also linked to an increased risk of immunologic failure.

Unemployment, substance use, and HIV stigma emerged as particularly salient correlates of psychological morbidity in this population.

Summary: The study found that in addition to physical health, pYLHIV (adolescents/young people living with HIV) also experience emotional depression and anxiety, requiring more psychological support to reduce their stress and improve self-esteem. Furthermore, HIV treatment and transmission knowledge alone are not sufficient to improve the self-esteem or reduce stigma of pYLHIV, indicating a need for integrated mental and physical health care.

Stakeholders generally found it acceptable, but identified challenges in managing depression along with HIV care, including organizational and patient-related barriers. Addressing these challenges would require changes at both organizational and patient levels for successful integration.
