Prevalence and clinical characteristics of sleep-disordered breathing in patients with heart failure of different left ventricular ejection fractions

Tao Wang et al.

Summary: This study investigated the prevalence and clinical characteristics of sleep-disordered breathing (SDB) in patients with heart failure (HF) with different left ventricular ejection fractions (LVEF). The results showed that SDB was common in HF, and the prevalence of SDB differed among patients with different LVEF, mainly due to differences in cardiac functions. The prevalence and severity of SDB were significantly higher in HF with reduced EF (HFrEF) and HF with mid-range EF (HFmrEF) compared to HF with preserved EF (HFpEF), primarily due to an increase in central sleep apnea (CSA). LVEF could predict the occurrence of CSA and SDB to a certain extent.

Summary: SGLT2 inhibitors have beneficial effects not only in improving diabetes compensation, but also in kidney and cardiovascular outcomes. These benefits are not limited to diabetics, but also found in non-diabetic individuals. The pathophysiological pathways underlying the treatment success have been investigated in clinical and experimental studies.

Summary: This paper provides an in-depth review of the relationship between HFpEF and NAFLD, proposing three NAFLD-related HFpEF phenotypes and discussing diagnostic challenges and treatment options. The authors suggest that NAFLD-related HFpEF should be recognized as a distinct HFpEF phenotype.

Possible Preventative/Rehabilitative Role of Gliflozins in OSA and T2DM. A Systematic Literature Review-Based Hypothesis

Vincenzo Maria Monda et al.

Summary: This review examines the relationship between obstructive sleep apnoea (OSA) and type 2 diabetes mellitus (T2DM), suggesting the potential use of sodium-glucose co-transporter-2 inhibitors (SGLT-2i) as a treatment target. It also proposes that SGLT-2i indications could expand beyond current ones, including glucose, lipids, uric acid, blood pressure, and body weight control, as well as chronic heart failure and kidney disease prevention.

Female Patients With Sleep-Disordered Breathing Display More Frequently Heart Failure With Preserved Ejection Fraction

Simon Lebek et al.

Summary: The study revealed that in patients with SDB, HFpEF and diastolic dysfunction were more common in women than in men, and the severity of SDB was associated with the degree of diastolic dysfunction in women. This suggests the need for sex-specific therapies for patients with sleep-disordered breathing and heart failure.

Obstructive sleep apnoea, intermittent hypoxia and heart failure with a preserved ejection fraction

John E. Sanderson et al.

Summary: OSA is recognized as a significant risk factor for HFpEF due to similar pathological effects and shared pathophysiological consequences, including activation of the sympathetic nervous system, renin-angiotensin-aldosterone system, systemic inflammatory state, and ultimately leading to myocardial fibrosis and structural changes that cause myocardial stiffening. Further trials of preventive treatment for OSA as a risk factor for HFpEF should be considered.
