Treatment of inflammatory bone loss in periodontitis by stem cell-derived exosomes

Summary: Exosomes secreted by healthy periodontal ligament stem cells (PDLSCs) promote osteogenic differentiation of PDLSCs derived from periodontitis tissue. Treatment with exosomes can accelerate bone formation in rat models of periodontitis. Mechanistically, exosomes suppress over-activation of the canonical Wnt signaling pathway to recover the osteogenic differentiation capacity of inflammatory PDLSCs. These findings suggest that MSC-derived exosomes are an effective and practical cell-free therapeutic for the treatment of periodontitis.

Mesenchymal Stem Cell Exosomes Promote Functional Osteochondral Repair in a Clinically Relevant Porcine Model

Summary: This study validates the safety and efficacy of human mesenchymal stem cell (MSC) exosomes combined with hyaluronic acid (HA) for osteochondral repair in a clinically relevant micropig model. The results show that the combination treatment promotes functional cartilage and subchondral bone repair, with significantly improved morphological, histological, and biomechanical outcomes. No adverse response or major systemic alterations were observed, indicating the potential of this therapy for clinical applications.

Potential Effects of Exosomes and their MicroRNA Carrier on Osteoporosis

Summary: Osteoporosis is a common skeletal illness characterized by bone production weakness and increased bone resorption, mainly affecting postmenopausal women. Although anti-osteoporosis drugs have been developed, treatment side effects still exist, calling for more reasonable therapeutic strategies. Exosomes, nanosized extracellular vesicles secreted by various cells, have advantages of lower toxicity and immunogenicity and contain functional molecules that can regulate signaling pathways and influence physiological and pathological behavior. This review summarizes the therapeutic potential of exosomal miRNAs derived from different cell types in osteoporosis and proposes a prospect of using new biomaterial scaffolds combined with exosomes for treatment.

Higher yield and enhanced therapeutic effects of exosomes derived from MSCs in hydrogel-assisted 3D culture system for bone regeneration

Summary: In this study, a 3D culture model was engineered to enhance the osteoinductivity of MSC-derived exosomes. Exosomes derived from 3D culture showed higher yield and improved efficiency of collection compared to conventional monolayer culture. Moreover, these exosomes significantly promoted osteogenic gene and protein expression, proliferation, and migration of bone marrow mesenchymal stem cells. Mechanistic studies revealed that the upregulation of the YAP signaling pathway is the underlying mechanism. The 3D-Exos also resulted in enhanced new bone formation in rats with alveolar bone defects.
