Antibiotic dosing adjustments in hospitalized patients with chronic kidney disease: a retrospective chart review

Summary: Infections in CKD patients are a major cause of hospitalization, with antibiotics frequently prescribed. However, our study in Lebanon found that over half of non-dialysis CKD patients received antibiotics without necessary renal dose adjustments. The most common inappropriately dosed class was penicillins, highlighting the importance of increasing awareness among prescribing physicians to prevent dosing errors.

Effect of Fluoroquinolone Use in Primary Care on the Development and Gradual Decay of Escherichia coli Resistance to Fluoroquinolones: A Matched Case-Control Study

Summary: This study found that patients taking fluoroquinolones (FQ) were at higher risk of colonization/infection by fluoroquinolone-resistant E. coli (QREC). The risk was highest in the first year after FQ treatment, decreased in the second year, and became undetectable thereafter. Older age, male sex, longer hospital stays, COPD, and diabetes were identified as independent risk factors for QREC colonization/infection. A single FQ course significantly increased the risk for at least two years, with higher risks in cases of longer hospital stays, COPD, diabetes, male gender, and older age. These findings could guide public health campaigns and prescriber education to promote rational antibiotic use.

Summary: The percentage of unnecessary antibiotic prescriptions in US physician offices and emergency departments decreased slightly from 30% in 2010-2011 to 28% in 2014-2015. However, there was a greater decrease in children from 32% to 19%, while unnecessary prescribing in adults remained unchanged.

A multifaceted intervention improves antibiotic stewardship for skin and soft tissues infections

Summary: The study assessed the effectiveness of a multifaceted stewardship intervention to reduce inappropriate antibiotic use in emergency department patients with skin and soft tissue infections. The intervention led to improved guideline adherence and decreased antibiotic duration in the intervention site compared to the control site. Adherence to guidelines was inversely associated with SSTI severity and purulence.
