Translational in vitro and in vivo PKPD modelling for apramycin against Gram-negative lung pathogens to facilitate prediction of human efficacious dose in pneumonia

Vincent Aranzana-Climent et al.

Summary: This study characterized the preclinical pharmacokinetics (PK) and pharmacodynamics of the drug candidate apramycin in fighting carbapenem-resistant gram-negative pathogens. A mathematical model was developed to predict human efficacy based on in vitro and in vivo data. Apramycin showed good lung penetration and the mathematical model integrated lung PK to drive mouse PK and pharmacodynamics. Apramycin was demonstrated as a candidate for treatment of carbapenem-resistant gram-negative pneumonia.

Pharmacokinetic/pharmacodynamic models for time courses of antibiotic effects

Iris K. Minichmayr et al.

Summary: Pharmacokinetic/pharmacodynamic (PKPD) models are valuable tools for characterizing and translating antibiotic effects. These models provide a more accurate description of the time course of antibiotic effects, considering mechanistic knowledge of pathogens and drugs. This review provides a comprehensive overview of previously published PKPD models, highlighting their details and main development purposes.

Clinical and pharmacokinetic/dynamic outcomes of prolonged infusions of beta-lactam antimicrobials: An overview of systematic reviews

Pierre Thabet et al.

Summary: This overview of reviews identifies a benefit in mortality and clinical cure with prolonged infusions of beta-lactams when compared with intermittent infusions. However, there is a need for more rigorous and transparent systematic reviews to confirm these findings, given the variability in populations, interventions, and outcomes among current systematic reviews.

From Therapeutic Drug Monitoring to Model-Informed Precision Dosing for Antibiotic

Sebastian G. Wicha et al.

Summary: Therapeutic drug monitoring (TDM) and model-informed precision dosing (MIPD) are crucial for individualized dosing in critically ill patients with infections. The use of MIPD software and new technologies has the potential to revolutionize antibiotic dosing, but more clinical trials are needed to validate their effectiveness.

Pivotal Role of Translation in Anti-Infective Development

Lena E. Friberg

Summary: The value of model-based translation in drug discovery and development is widely recognized, aiding in compound selection, dose prediction, and risk reduction. However, the importance of time-dependencies is often overlooked due to species-specific biological turnover rates. Models capturing the full time-course offer important quantitative information for rational decision making and treatment optimization.

Antibiotic pharmacokinetic/pharmacodynamic modelling: MIC pharmacodynamic indices and beyond

Gauri G. Rao et al.

Summary: The use of pharmacokinetic/pharmacodynamic approaches is crucial in optimizing dosage regimens of anti-infectives to maximize efficacy, minimize toxicity and resistance, and ensure a high likelihood of therapeutic success. This involves well-designed preclinical studies, mathematical-model-based analysis, in-silico simulations, and the integration with population pharmacokinetic modelling to consider interindividual variability and predict treatment outcomes in patients. The importance of this approach is highlighted through the example of Polymyxin B, showcasing the necessity of pharmacokinetic/pharmacodynamic modelling in conjunction with experimentation for successful translation to clinical practice.

Application of the hollow fibre infection model (HFIM) in antimicrobial development: a systematic review and recommendations of reporting

Zahra Sadouki et al.

Summary: This systematic review focused on the use of the in vitro hollow fibre infection model (HFIM) for microbial culture. The majority of publications lacked sufficient data to allow for results evaluation, indicating a need for improved reporting and laboratory standardization of HFIM experiments.
